The Mike O'Callaghan Federal Hospital, a 104-bed joint Air Force/Veterans Affairs facility, provides care to 44,000 DoD and eligible VA beneficiaries in the Las Vegas community. More than 1,400 personnel offer full spectrum preventive, emergency and acute care services.

The 99th Medical Group runs the facility, staffs all outpatient activities and manages 46 DoD beds. Squadrons under its auspices: the 99th Aerospace Medicine Squadron, 99th Dental Support Squadron, 99th Inpatient Squadron, 99th Medical Surgical Squadron, 99th Medical Operations Squadron and the 99th Medical Support Squadron. The VA manages 58 beds and cares for VA inpatients from the hospital's emergency room and other VA facilities in Las Vegas.

Robust inpatient and outpatient services include such key specialties as Family Medicine, Internal Medicine, Pediatrics, Orthopedic and Cardiology. Other inpatient services include medical/surgical wards, and maternity and intensive care units. The hospital also hosts several residency and teaching programs.

The Mike O'Callaghan Federal Hospital is an integral part of the federal health care system in the TRICARE West region. TRIWEST Healthcare Alliance, a managed care contractor, has developed a network of physicians and hospitals in Las Vegas for off-base care. TRICARE access to care standards include: address acute needs within 24 hours, make routine appointments within 7 days and preventive and wellness appointments within 28 days.

Visit the hospital's TRICARE Service Center for such questions as enrollment, disenrollment, provider listings and authorizations or call (888) 874-9378 or go to www.triwest.com. The hospital also staffs a full-time health benefits advisor to assist members with debt collection issues and to provide group information briefing upon request.

Enrollment in TRICARE Prime
To access primary care, you must enroll in TRICARE Prime and be assigned an MOFH Primary Care Manager. Specialty care may be available to all DoD beneficiaries, but locally enrolled TRICARE Prime members have priority. Active duty members and their families typically enroll in TRICARE Prime while in-processing at the base "Right Start" briefing.

Working with your personal Primary Care Manager
Shortly after submitting the TRICARE Prime enrollment forms, each enrollee receives a primary care manager -- a physician, physician assistant or nurse practitioner. Your PCM is supported by a team including a nurse, medical technicians, and administrative clerks. A computer system will reflect your PCM when you book appointments at (702) 653-2273 or www.tricareonline.com.

Please establish a positive working relationship with your PCM and proactively manage your care. Direct requests to change your PCM to the TRICARE Service Center or call (888) 874-9378.

Medical Records
The DoD uses an outpatient electronic health record. Visits documented in the electronic record can be accessed at any DoD medical treatment facility. They also maintain outpatient records on paper. These documents are property of the U.S. Government but you can review them any time in the presence of a staff member. You can also request copies of your medical records from the Patient Administration Office at (702) 653-3417 or (702) 653-2513.

Privacy of Patient Information
According to the federal Health Information Portability & Accountability Act, no personally identifiable information or details of your health care will ever be disclosed without your authorization. If you have questions regarding disclosure of medical information, please contact the TRICARE privacy officer at (702) 653-3417 or (702) 653-2513.

Patient Advocate Program
Each clinic within the hospital has a patient advocate assigned to help resolve issues at the lowest possible level. Simply ask any staff member to arrange a meeting with the clinic's patient advocate. From time to time the clinic-level advocate may not be able to resolve your concern. In those rare instances, visit the Hospital Patient Advocate in the TRICARE office on the first floor or call (702) 653-2201.

ID Cards
Access to health care services requires a valid uniformed services ID card and listing in the Defense Enrollment Eligibility Reporting System database. Children under age 10 can generally use a parent's or guardian's ID card, but they must be registered in DEERS. At age 10, the sponsor should obtain an ID card for the child.

Third Party Collections
Federal law requires patients to report any third-party insurance. The government may be able to bill your insurance company and recover costs associated with your care. There is no cost to you and, by law, third-party insurance companies cannot alter your coverage or raise your premiums based on the government's efforts to collect funds. In addition, claims filed by the government for care may count as payment toward a deductible on your private health plan. You will be asked about third-party coverage at least once per year. Please help us by having your third-party information handy when you visit.

Off-Base Referrals
The MOFH scope of care is quite extensive, but patients occasionally require a provider in the local Las Vegas area. Typically your PCM arranges care through the Referral Management Center. A week after your PCM visit, you will receive an authorization letter from TriWest designating your provider. Please ensure that your referring provider has your contact information in the computer system to minimize notification delays. You can choose any network provider. If you wish to change your specialty provider, please call (888) 874-9378 to ensure that claims are paid properly. Referral results are returned directly to the referring provider.

Preventive Services
Be proactive in managing your health care and work closely with your PCM to take care of all preventive services, especially such critical items as pap smears, mammograms, immunizations and prostate exams.

The Internet sites as such as www.WebMD.com offer credible information about health care concerns. Also, self-care books are available at most bookstores. The TRICARE office can also order a self-care book for you upon request. Be sure to take advantage of patient educational materials found in lobbies and common areas.

Personnel Reliability Program Procedures
Nellis AFB takes its PRP responsibilities seriously. Patients on PRP status should always identify themselves to ensure that any care won't hamper their ability to perform sensitive duties. All PRP medical records are kept in a separate location and require additional safeguards to access your information. TRICARE Access to Care Standards
Enrollment in TRICARE Prime allows you to take advantage of TRICARE Access to Care Standards. In a nutshell, you should be able to access urgent care within 24 hours, routine care within seven days and specialty care within 30 days. Any situations outside those standards will require alternate arrangements.

The hospital has a centralized telephone appointment system. Most hospital appointments are scheduled, cancelled and/or verified by calling (702) 653-2273. This number will activate a voice system, which will direct you to the needed medical section. In some cases, you will need to dial the clinic directly, so please be prepared to write down the phone number given. Active duty and TRICARE Prime patients have priority in scheduling appointments. Appointments with most specialty clinics require a referral. NOTE: Members on flying status may call the Flight Medicine Clinic directly to schedule appointments at (702) 653-3002 or (702) 653-2766.

The Family Medicine Residency Clinic offers a broad range of services to people of all ages, birth to geriatrics, as we strive to be your "personal medical home," both in the clinic and on the inpatient wards. Services offered include primary care, well baby and child care, sports physicals, walk-in suture check/suture removal clinic, walk-in nurse-run clinic, minor skin surgery, colonoscopy, vasectomy, acupuncture, osteopathic manipulation, cardiac treadmill testing and cervical colposcopy. We perform all active duty preventive health assessments, as well as pre and post deployment evaluations. We serve all non-flying active duty, retirees and dependents. We are staffed by Family Medicine physicians, Family Medicine resident physicians in training, active duty/GS/contractor family physicians, physician assistants and nurse practitioners.

Miscellaneous Issues:
• Quarters - Per AFI 41-210 section 3.6.4: Unit commanders and supervisors have the authority to grant up to 24 hours sick status at their discretion. If the illness/injury persists beyond 24 hours and still interferes with completion of duties, then the commander or supervisor must refer the member to the Military Treatment Facility for treatment and subsequent quarters authorization.

• Convalescent Leave - all requests for convalescent leave from off-base providers need to be processed by your Primary Care Manager. Please bring off-base requests to your PCM.

Pediatrics Clinic

7:30 a.m. to 4:30 p.m. Monday through Friday The Pediatrics Clinic meets acute, routine and specialty pediatric needs. The pediatrics staff is composed of highly skilled board certified pediatricians, certified nurse practitioners, RNs, qualified medical technicians and medical assistants. The clinic staff are strong supporters of putting prevention into practice. A large selection of health education brochures and health tips are available. Services available include pediatricians on-call after hours, well baby checks, breastfeeding support, sports physicals, walk-in suture check/suture removal clinic and a walk-in strep throat clinic (please call ahead).

Nellis AFB Pediatric Clinic is proud to be working together with the Reach Out and Read Program, a national program focused on improving literacy at age-appropriate levels; and with Baby Friendly Birth Facility, which focuses on supporting breastfeeding mothers and infants.

Internal Medicine Clinic

The Internal Medicine Clinic provides an ambulatory care setting for initial patient evaluations and ongoing care for enrolled adult patients with problems such as, but not limited to, the following diseases and disorders: cardiovascular, metabolic, hematology/oncology, gastrointestinal, pulmonary, neuromuscular, renal and rheumatologic.

For those conditions considered beyond the scope of general internal medicine, patients will be referred to a network specialist. In addition, consultation and evaluation of emergent patients whose problems fall within the realm of internal medicine are provided at the request of the emergency services physician.

Enrolled patients requiring hospitalization are cared for by a general internist and admitted to the Air Force Medical Surgical Unit or Intensive Care Unit. Patients who require assumption of total care from a medical sub-specialist not available at this hospital are transferred to a local medical facility capable of providing the necessary service.

Flight Medicine Clinic

The Flight Medicine Clinic is in a standalone building on the main base, next to the Enlisted Club and across the street from the base theater. The Flight Medicine Clinic provides routine medical care to the base flying population. Personnel eligible for medical care include pilots, navigators, air traffic controllers, flight engineers and all personnel who hold a 1042 clearance. Active duty sick call for flyers is from 7:30 a.m. to 8 a.m. and 1 p.m. to 1:30 p.m. Routine medical appointments can be made weekdays from 7 a.m. to 4:30 p.m. by calling the appointment line at (702) 653-3002. After duty hours, flyers should go to the MOFH emergency room. The flight surgeon on call will be notified of ER visits by flying personnel. Should a flyer be seen outside of the MOFH, it is their responsibility to notify the flight surgeon at the first Sick Call following the visit. Flyers may NOT fly until cleared by a flight surgeon after receiving treatment in the ER or from a provider outside the hospital.

Creech Medical Aid Station

The Creech Medical Aid Station provides full flight physicals and physical health assessment examinations, drug urinalysis testing, gas mask fit testing and sick call services. Dental examination and cleaning services are also available.

Emergency Services

Emergency services are available 24 hours a day. An emergency is defined as an acute illness or injury that may pose a threat to life, limb or eyesight; results in unreasonable pain and suffering; or requires immediate care or attention to ensure the best possible recovery. Report directly to the emergency room for evaluation or call 911 if life threatening. Evaluation by a provider is based on triage category (or severity of illness or injury). Patients who present with non-urgent problems may be referred to the Family Practice, Pediatric, Internal Medicine and Flight Medicine Clinics for evaluation and treatment. Ambulance service is available within the boundaries of Nellis AFB.

When requesting an ambulance:
• Patients requiring an ambulance from anywhere on base and those residing in base housing can call 911
• Patients requiring an ambulance outside these areas can contact a civilian ambulance by calling 911 If you call 911 from a land-line on base, you will be contacting Nellis AFB Emergency Services.

If you call 911 from a cell phone, you will be contacting Clark County Emergency Services. They may redirect your call to Nellis AFB Emergency Services, or they may respond via ambulance.

New handwritten prescriptions may be filled at the satellite pharmacy. Prescriptions from civilian providers will be filled if the medication is on the hospital's formulary—a listing of stocked medication.

Refills are available through a phone-in refill system and should be picked up at the drive-through windows at the satellite pharmacy. Listen to the automated message for changes in hours of operation, formulary changes and other important pharmacy information.

Dental Services

General dentistry and selected specialty services are provided for active duty personnel. Routine appointments are made following an examination to determine the type and extent of care required. Periodic dental exams and cleanings are coordinated through unit monitors/schedulers. Members desiring exams or cleanings beyond that offered with the periodic exam may request these services through the clinic. Dental treatment (other than Nellis Dental Clinic referrals) obtained from a civilian dentist, including elective care such as orthodontics, must be paid for by the member and coordinated with the clinic prior to initiating treatment. Nellis is home to a variety of dental residency programs.

Emergency Dental Care
Emergency dental care is defined as uncontrolled pain, bleeding or rapid swelling/significant trauma to the oral-facial structures. Immediate care is available for all beneficiary categories. Sick call is held in the clinic at 7:30 a.m. and 12:30 p.m. on duty days by appointment; active duty members have priority. A dentist and technician are on call after duty hours, including family days, weekends and holidays, for consultation and management of dental emergencies. Patients must report to the emergency room during these times for initial triage and management.

Space-Available Care
Family members and retired military personnel receive treatment on a standby, space-available basis only. Treatment will be limited to exams, cleanings and other single-procedure visits. Standby, space-available care has been significantly curtailed by DoD and Air Force policy and is extremely limited. Care guidelines are available at the clinic reception desk. Non-active duty members are encouraged to seek care in the private sector.

Active duty members are encouraged to enroll their families in the TRICARE Active Duty Family Member Dental Plan to ensure routine and emergency treatment needs are provided through civilian sources. Enrollment in the dental plan is done through Customer Service at the Military Personnel Flight.

Optometry Services
Optometry Clinic

Mike O'Callaghan Federal Hospital, Lower Level of Hospital Monday-Friday, 7:30 a.m. to 4:30 p.m. Active Duty, TRICARE Prime and TRICARE for Life members are eligible for appointments in the Optometry Clinic. However, due to staff deployments and manning, appointments can, on occasion, be limited to active duty only. Patients are required to call the Appointments System at (702) 653-CARE (2273) to make their routine eye appointments. Acute problems such as pink eye (conjunctivitis), abrasions, foreign bodies, flashes and floaters, referrals and/or follow-ups from the Emergency Room or other clinics in the hospitals can be seen on a walk-in basis.

Routine eye appointments are provided for examination of eye health and update of eyeglasses. Patients should bring to this appointment their most current eyeglasses and all eye medications they are currently taking. Patients' eyes can be dilated during this appointment making distance and near vision difficult. Having the most current eyeglasses and sunglasses available can alleviate some of the discomfort of dilation. Patients should bring a driver with them if they believe they may have problems driving after having their eyes dilated.

Glasses - Only active duty and retirees are eligible for glasses. Glasses can be ordered through the Optometry Clinic by presenting an eyeglass prescription that is no more than a year old. If the prescription is greater than one year old, a routine eye appointment should be made before glasses can be reordered.

Contact lens services - New contact lens fittings are only provided to personnel on the Aircrew Soft Contact Lens Program and those with medically indicated conditions (e.g. Keratoconus). Patients who are not on the Aircrew Soft Contact Lens Program or have any medically indicated conditions are eligible for Contact Lens Updates if they already are wearing contact lenses and knowledgeable on insertion, removal, wear and care of their contacts. Contact lens services are provided during the routine eye appointment. Patients desiring contact lens services must call Central Appointments, at (702) 653-CARE (2273), and book for a routine eye appointment. When presenting for the routine eye appointment, the patient must wear their contact lenses to the appointment and have a current copy of their contact lens prescription available. If the patient cannot locate the written prescription, the patient can bring in the packaging/ box that their contact lenses came in that has the contact lens prescription on it. Since the patient's lenses are used as the fitting lens, the patient must be able to provide those parameters to the doctor before an update can be written. If an optometrist cannot correct the patient to 20/20 with the patient's lenses, the patient will be told that their present lenses cannot fully correct them and a refit elsewhere with different lens parameter will be needed. As with the routine eye appointment, patients should bring with them their eyeglasses and all eye medications they are taking.

Acute eye infections/trauma - Acute eye infections such as conjunctivitis (pink eye), abrasions, foreign bodies, flashes and floaters, or sudden vision loss can be seen by the Optometry Clinic on a walk-in basis. It is preferable to present to the clinic these problems first thing in the morning. Please contact the clinic at (702) 653-3010 and let us know that you want to be seen. If you are in the hospital, stop by the Optometry Clinic and request care. If you have been seen elsewhere for the same problem, please bring in any medications previously prescribed and documentations received.

Refractive surgery - Photorefractive Keratectomy, or PRK, and Laser Assisted In-Situ Keratomileusis - LASIK - are available for active duty members only. The Optometry Clinic provides pre-op evaluations and post-op care. The actual refractive surgery is not accomplished in the Optometry Clinic. Sites for the refractive surgery can be located on the website below. This program is not approved for active duty members under the age of 21 years. This program requires a full eye exam performed within the last year prior to the laser surgery work-up to determine the patient's refractive error stability. Patients wearing contact lenses must be off their contact lenses 30 days prior to the laser surgery workup. Patients should schedule for a routine eye appointment first to determine if they are a candidate before submitting their application for laser surgery. Health and Wellness Center (HAWC)

The mission of the Health and Wellness Center is to maintain and improve the physical, social and mental well being of the Nellis community. The HAWC targets wellness in fitness, nutrition, tobacco use cessation, stress management, substance abuse and other related preventive programs. Upon referral or individual request, individuals can be evaluated and receive counseling by experts at the HAWC.

Services include fitness assessments, nutrition education, stress management, education on cardiovascular disease and cancer prevention. All eligible beneficiaries and Department of Defense civilians may participate in program services. For tobacco use cessation, eligible beneficiaries have two methods to quit the habit. The first method is to call the American Lung Association Quit Line Program at 1-800-548-8252. The ALA has registered nurses and respiratory therapists to guide participants through the steps necessary for ending tobacco use. The second method is Tobacco Cessation classes offered every Thursday at 4 p.m. The course consists of five class sessions to include Stress Management, Weight Control, Medical Aspects of Smoking, How to Quit and Relapse Prevention. HAWC personnel are available to come to your squadron to present briefings on various health topics.

The Public Health Flight handles many diverse programs to ensure a healthy Nellis community. Members of this flight perform deployment medical clearances, medical profile management, hospital employee health, inspections of Nellis' food establishments, communicable and sexually transmitted disease screening and manage the base occupational health, rabies and tuberculosis screening programs.

The Bioenvironmental Engineering Flight provides industrial hygiene and radiation protection surveillance for industrial shops on base. BEF evaluates such hazards as noise, industrial chemical use, ionizing and non-ionizing radiation, biological hazards, ergonomics and indoor air quality, and strives to ensure that commanders and supervisors provide their workers with a safe and healthful workplace. In addition, BEF monitors the quality of base drinking water. BEF manages industrial respirator and gas mask fit testing, issues thermoluminescent dosimeters for radiation exposure dose records and plays a principal role in base emergency response to plane crashes, chemical releases, etc.

Veterinary Services (U.S. Army)

The Nellis U.S. Army Veterinary Treatment Facility offers limited veterinary care for family pets. Services routinely available are vaccinations for dogs and cats, testing for intestinal parasites and health certificate examinations. This facility does not provide surgical services, hospitalization, boarding or grooming. Pets are vaccinated by appointment only. Payment is required in the form of cash or check, Visa or MasterCard at the time the service is rendered.
