Registering Families in Quality Child and Youth ProgramsImportant Changes to Kentucky Child Care Council (3C's) Assistance Program.Please click here and take a moment to fill out a quick survey.Fort Knox Parent & Outreach Services connects Families to Child, Youth, and School Services programs through Central Enrollment, Special Needs Accommodations Process (SNAP), Kids On Site Child Care (KOS), Soldier Family Assistance Center Child Care(SFAC), Playgroups, Parent Participation Plan, Special Openings, Annual Parent Plan, and Volunteer Opportunities.Army Family Covenant/Deployment Benefits DOWNLOADEligibilityEligible users include dependents of Active Duty Military, National Guardsmen, Reservists, Retirees, Department of Defense Civilian employees, and Contractors who register with CYSS Parent Central Services.Registration and FeesFREE registration can be processed at:Parent Central ServicesBuilding 13841384 Chaffee Ave.Fort Knox, KY 40121(502) 624-6703Walk-in Registrations are available Monday through Friday, 0900-1200 Registrations by appointment are available Monday through Friday, 1300-1500 Late appointments are available Thursday or upon request* Parent Central Services is closed the 3rd Thursday, each month, 1300-1630 for staff training.To expidite your CYSS Registration, please have the following available.THE FOLLOWING FORMS ARE AVAILABLE ON WEBTRACCompleted Registration Packet (Infant-5th Grade or 6th-12th Grade) for new registrations or Completed Re-registration Packet Proof of Eligibility (Sponsor/Child Military ID card or Guardianship documents) Child's Official Immunization Record: The CYSS/DoDEA Immunization Requirements MUST BE USED Two Emergency Contacts (name, address, telephone number) LES and/ or pay statement and recent W2 form Health Assessment or well baby check up (due within 30 days of registration) A current Sports Physical must be presented before signing up for Youth Sports Current Family Care Plan for single or dual military Families Health Screening Tool Annual registration fee is waived in support of the Army's Commitment to Families through the Army Family CovenantChildren with Special Needs: Admission criteria for children/youth with Special Needs: Parents are responsible for informing PCS registration of special needs, health conditions and/or medications /services their child/youth may be receiving. Parents must complete an Army CYSS Health Screening Tool. After review of your Health Screening Tool, PCS or a CYSS Nurse may contact you and require additional forms to be completed in regard to special needs.CYSS services will not be available until a Special Needs Accommodation Process (SNAP) team reviews all forms and approves services.Requirements for Special Needs Accommodation Process DOWNLOADOther PCS Services: In/Out Processing Services, Global Data Transfers (import/export CYMS file), Waiting List information, Resource and Referral (to other installations and Family Child Care). Visit Army FCC Online to learn more about FCC providers.Fee Adjustments for Financial Hardship/extenuating circumstances: The Garrison Commander or delegated designee may adjust fees for individual Families based on financial hardship or other special circumstances on a case by case basis. Families must demonstrate a need for childcare fee reduction based on a review by an Army Community Services financial counselor. Financial/extenuating circumstances hardship must be reviewed and approved by the Commander or designee.Parent involvement opportunities and Volunteers: Parents are encouraged to participate in all aspects of CYSS programs. Parents may volunteer in CYS Services as coaches, assisting with craft projects, reading, sharing talents, or by attending Parent meetings. Contact your Program Director if you would like to volunteer.Parent Participation Program: Parents may earn a fee reduction for volunteering a minimum of 10 hours and earning Parent Participation points in a CYS Services full day, part-day and party-time program. Participation may occur in their child’s program or another CYS Services program. Participation points may be accumulated from month to month until the Parent earns 10 hours to receive a 10 percent reduction on one month’s fees for one child.CYSS Parent Participation Program DOWNLOADCYSS Annual Parent Plan DOWNLOADParent Participation Program Opportunities (CDC) DOWNLOADParent Participation Program Opportunities (SAC) DOWNLOADPlaygroup: The Fort Knox Community offers a free Playgroup which encourages Parent and Child interactions through unstructured activities for children up to 5 years of age. This is an opportunity for parents to enjoy activities with your child and meet other parents. It is held at the Devers Middle School & Teen Program on Fridays from 1000 to 1130 when school is in session.FY 13 CYSS Playgroup VIEW FLYERKids On Site: Used for on-site childcare during meetings and social gatherings that meet the needs of the community. These events may include, but are not limited to: Community Spouse's Club Events, Volunteer Appreciation Luncheons, Religious Functions, and Family Readiness Groups. Child, Youth, and School Services provide the staff to care for children between the ages 6 weeks through 5th Grade during the event. Kids on Site increases the capability of Child Development Services Hourly Care Program, as well as, making meeting attendance more attractive to parents. On-site care is convenient for parents and all childcare staff members are trained and qualified through CYSS.More information about Kids On SiteSoldier Family Assistance Center: Child, Youth and School Services provides on-site childcare for the the Fort Knox Soldier & Family Assistance Center (SFAC), during medical appointments that meet the needs of the Wounded Warrior. Child care is available for children between the ages of 6 weeks through 5th grade. The SFAC is located on the Warrior Transition Battalion (WTB) campus and has a state of the art design, showcasing a developmentally age appropriate and safe environment. Click for more information
