E Komo Mai (Welcome) to Naval Health Clinic Hawaii (NHCH) where your health and readiness are our focus. NHCH is comprised of two large clinics, Makalapa at Joint Base Pearl Harbor-Hickam (JBPHH) and Kaneohe Bay at Marine Corps Base Hawaii.


Other smaller clinics are located at Camp H.M. Smith, Wahiawa, and Barking Sands on the island of Kauai. Of note, the headquarters for NHCH is not co-located with the clinics, but resides in Bldg. 1750 at 480 Central Ave. adjacent to the Pearl Harbor Naval Shipyard. Preventive Medicine, Industrial Hygiene and Occupational Health Services are provided at this site, which is also known as the Shipyard Clinic.


NHCH serves active-duty personnel, their family members and retirees. Services provided include family medicine, pediatrics, obstetrics/gynecology, immunizations, comprehensive dental care, dermatology, optometry, preventive medicine, physical therapy, chiropractics, mental health care, deployment health, aviation medicine, undersea medicine and a 24-hour primary care manager. Ancillary services include pharmacy, radiology and laboratory. NHCH also provides occupational health services to civilian employees.


Enrolling to Naval
Health Clinic Hawaii
Medical care at NHCH begins with the Tricare Service Centers (TSC), where active- duty members, family members and retired beneficiaries begin their check-in process to enroll to NHCH for care. Staff will assist with Tricare enrollment, Primary Care Manager (PCM) assignment, registration into CHCS/AHLTA and Medical Record updates. Active-duty members will also be directed to complete the Periodic Health Assessment (PHA), part of the Individual Medical Readiness process. Staff to include health benefits advisors are available to answer Tricare and enrollment questions.

During the check-in process, information about several resources, including Tricare Dental, Tricare Online, Exceptional Family Member Program and Advance Directives are provided.


Please bring ID cards, social security numbers (for all enrolling), medical records, dental records (active duty only) and immunization records when you check in. A Request for Medical/Dental Records, DD Form 877, will facilitate this process if completed prior to moving to Hawaii. This form is available from your current military treatment facility (MTF) Medical Records Department. If you do not have your medical record, medical records will request it to be sent from your last provider. You can still complete the check-in process. If you have never had a military health care system medical record, we will make one for you. For further information about the check-in process, please call the Health Benefits Advisor.

A walk-in service center for information and assistance with your Tricare contract such as enrollment, benefits, claims and authorizations.

Health Benefits Advisor
NHCH Health Benefits Advisor (HBA) is located at Makalapa Clinic (Bldg. 1504), phone (808) 473-2444, ext. 509, weekdays 8 a.m.-4 p.m.

Tricare Prime at NHCH
Choosing to enroll in Tricare Prime at NHCH guarantees certain benefits, including: timely and priority access to care, coverage away from home, preventive medical services, a Primary Care Manager (PCM) to coordinate your health care, direct communication with your PCM via NHCH Online (powered by RelayHealth), appointment scheduling via NHCH Central Appointments, sports physicals for children and adolescents, access to online PCM appointments with Tricare Online.


Exceptional Family
Member Program (EFMP)
The Exceptional Family Member Program (EFMP) identifies and documents the special medical, behavioral health and/or educational needs of family members to ensure that those needs are considered during the assignment process. Enrollment in EFMP is mandatory for all family members of active-duty service members with needs requiring specialized services. NHCH EFMP Coordinator is located at the Joint Patient Liaison Office, Tripler Army Medical Center, Wing B, Room 1B101. Telephone: 433-9644, weekdays, 7:30 a.m.-4 p.m.


Tricare Dental Program
Information on the Tricare Dental Program may be obtained directly from Metropolitan Life (MetLife) Insurance Company by visiting their website https://employeedental.metlife.com/dental/public/EmpEntry.do or by calling (855) 638-8371. The Tricare Family Member Dental Plan is a dental insurance plan for family members of active-duty military to receive dental care with civilian dentists. All family member dental care is provided within the civilian community.


To ensure your family members’ dental needs are met, we encourage active-duty military to enroll them into the Tricare Family Member Dental Plan (TFMDP). This will avoid costly dental bills. For more information, see your Health Benefits Advisor Office, or contact the MetLife office at https://employeedental.metlife.com/dental/public/EmpEntry.do or call (855) 638-8371. Military retirees may call (888) 838-8737 to enroll in Tricare Retiree Dental Plan or view/enroll at www.trdp.org.

If you need to speak with a medical provider anytime the clinics are closed, simply call
473-0247, ext. 3 and you will be connected to an answering service who will contact the on-call provider. The provider will be able to help you decide how to treat an illness after hours or whether you should go the Emergency Room. Please note the provider has no access to labs, X-rays, appointments or medical records.


Makalapa Clinic (JBPHH)
Urgent Care Walk-in Clinic
Offers NHCH Tricare Prime beneficiaries the convenience of being treated for an acute, non-emergent medical illness or injury on Saturdays during the times listed below without having to call for an appointment.


Urgent Care Walk-in Clinic hours are: Sat., 8-11 a.m. and 1-3 p.m.


What’s Urgent Care? Urgent care services are medically necessary services which are required for an illness or injury that would not result in further disability or death if not treated immediately, but require professional attention and have the potential to develop such a threat if treatment is delayed longer than 24 hours. An urgent care condition could be a sprain, sore throat or rising temperature. Urgent care services are provided by Physicians, Physician Assistants and/or Nurse Practitioners.


Emergency Services
Dial 911 or report to Tripler Army Medical Center Emergency Department.


For life-threatening emergencies, report to nearest Emergency Department.


What’s an Emergency? Tricare defines an emergency as a medical, maternity or psychiatric condition that would lead a “prudent lay person” to believe that a serious medical condition exists, or that the absence of immediate medical attention would result in a threat to life, limb or sight, and requires immediate medical treatment or which has painful symptoms requiring immediate attention to relieve suffering. For emergencies, such as profuse bleedings, unconsciousness, chest pain and obviously broken bones, dial 911, go to Tripler Army Medical Center or the nearest emergency room.
