REFERRAL MANAGEMENT OFFICE
The Referral Management Office (RMO) is responsible for facilitating the referral process for all Active Duty Service Members and TRICARE Prime beneficiaries enrolled to Reynolds Army Community Hospital. The hours of the RMO are 7:30 a.m. to 4:30 p.m. It is located adjacent to the TRICARE Service Center on the first floor of the hospital near the south entrance. You may also reach a staff member by calling (580) 558-2000, option 4, during these hours.
How the Referral Process Works
Your Primary Care Manager (PCM) is responsible for directing all of your health care needs. If a referral is needed to a specialty care provider, your PCM will enter a referral through the electronic medical record. The majority of referrals are processed by the staff of the Referral Management Office. Within 24 hours, a registered nurse will review your referral and make the determination of where best to obtain the specialty care your PCM has requested. Although the first choice is always Reynolds, our facility may not be available depending on current capability and capacity at our specialty clinics. If the service is available inside RACH, you will be called by one of the RMO staff to assist you with scheduling your appointment, generally within 48 hours from when it was ordered. To ensure that a member of the RMO is able to reach you, please stop by any clinic front desk to update your address and current phone numbers. If you have not received a call within five days from when your referral was ordered, please call the RMO.
Appointments Scheduled with a Network Provider
If the specialty care your PCM requested is not available within RACH, the RMO staff will coordinate with the TRICARE contractor to obtain an authorization with a civilian network provider for your specialty care needs. Once the authorization is obtained, one of the RMO staff will contact you to assist you with scheduling an appointment with the assigned civilian provider. Humana Military will also mail you an authorization letter to your current address as listed in DEERS. This letter will detail who the authorized provider is, how many visits you are authorized, and will specify a length of time that the authorization is valid. If you choose to make your own civilian appointment after receiving your authorization letter, please contact the RMO office so that they will be able to notify your PCM of your anticipated appointment date. This will assist us with obtaining the consultation report from your civilian specialist. Please keep a copy of this letter and note the date that your authorization expires. If a follow up appointment is needed and your authorization has expired, a new referral will be required from your PCM. Please contact the referral office directly and they will work with your PCM to renew your referral.
Do I Need to Take My
Records to the Civilian Provider?
One of the services offered by the RMO is that they will fax a copy of your pertinent records to the authorized civilian provider. If you had a related X-ray or MRI, please contact radiology to obtain a copy to take with you to your civilian appointment.
Case Managers
Your provider may refer you to a Case Manager if you have certain healthcare management needs. Case managers assist patients and families in coordinating unique, chronic, and catastrophic healthcare services; including appointments with specialists, hospitals and other agencies such as home health, hospice or medical equipment companies. If you would like more information please visit the Medical Management Division, located adjacent to the TRICARE Service Center near the south entrance of the hospital. Specialty Care Services Initial access to specialty care service requires a referral or provider order (see Chapter 5). To schedule a follow-up appointment, call (580) 558 2000, option 3, Monday through Friday. All clinics are open 7:30 a.m. to 4:30 p.m. unless otherwise noted. AUDIOLOGY CLINIC Soldiers in need of a hearing test should coordinate with their unit Hearing Conservation Officer to schedule an appointment. Results are provided on a same day basis. Routine audiology testing is available on a referral basis. CHIROPRACTIC CLINIC Chiropractic services are limited to Active Duty Soldiers only, by mandate from the Department of Defense. The clinic is located on the first floor by the west entrance, inside the Physical Therapy Clinic. DERMATOLOGY CLINIC This clinic offers full medical and surgical dermatologic services and provides diagnosis and treatment for both malignant and non-malignant conditions of the skin. GENERAL SURGERY CLINIC The General Surgery Clinic provides comprehensive care, pre-operative evaluation, operative and post-operative care, for a number of disease processes to include, but not limited to, breast disorders, endocrine disorders, abdominal disorders, hernias, pancreatic disorders, pediatric surgery and surgical endoscopy. Bariatric surgery is also available at RACH. Services include laparoscopic adjustable gastric band, sleeve gastrectomy and gastric bypass. Every patient requesting bariatric surgery needs to be evaluated by behavioral health, a dietician and a primary care provider. The clinic provides monthly bariatric surgery support group meetings. NEUROLOGY CLINIC The neurology clinic provides comprehensive, non-surgical treatment of diseases and disorders of the central and peripheral systems and EEG's are also performed in this clinic. OB/GYN CLINIC This clinic provides family-centered care for pregnant moms and comprehensive women's health care services. Patients may walk-in for pregnancy tests during clinic hours from 7:30 a.m. to noon and 1 to 4 p.m. The clinic runs a Centering Pregnancy Program that offers group support and education to pregnant women. Groups meet for two hours with a provider and/or a midwife for their monthly OB checks. During this visit, instructors offer detailed discussion on topics such as nutrition, pregnancy problems, family and parenting issues. Call (580) 558 2276/3069 for more information. OCCUPATIONAL THERAPY CLINIC Occupational therapy is a rehabilitative service encompassing screening, evaluating, treatment and education of persons who suffer from upper extremity orthopedic problems, neurological conditions and physical or psychological dysfunction relating to general and surgical conditions. Services include clinical assessment and treatment, life skills training and therapeutic exercise. Walk-ins are seen as schedules permit. OPHTHALMOLOGY CLINIC This service provides a broad spectrum of eye care and surgical procedures. When complex or unusually serious eye diseases are encountered, the Ophthalmologist works with TRICARE network sub-specialists located in Oklahoma City to provide the best care possible for our patients. OPTOMETRY CLINIC Active Duty Soldiers can self-refer for routine eye exams by calling (580) 558-2000, option 3. This clinic does not provide initial fittings for contact lenses but can often renew contact lens prescriptions. Corrective laser surgery is not offered at Reynolds; however, Active Duty may obtain information about the Army's Refractive Surgery Initiative. Family Members and Retirees are seen in the optometry clinic when space is available. Information about space available care or self referral to a network optometrist for Family Members and Retirees can be obtained at the clinic or the TRICARE Service Center. ORAL SURGERY The Oral Surgery Clinic provides surgical correction of developmental jaw deformities for Active Duty Soldiers. Surgeries include but are not limited to head and neck trauma management, facial reconstruction, dental and facial implants. ORTHOPEDICS/PODIATRY/ PHYSICAL MEDICINE CLINIC This clinic provides care for routine orthopedics, general podiatry, physical medicine, chronic pain management, rehabilitation, sports medicine, casting, acute trauma and emergencies. The Orthopedics/Podiatry/Physical Medicine Clinic also provides operative care for knees, shoulders, hands and feet, hips, soft tissue and orthopedic injuries. OTOLARYNGOLOGY/ HEAD AND NECK (ENT) This service provides medical and surgical treatment for patients with diseases and disorders of the ear, nose, throat and related structures of the head and neck. The Otolaryngologist's special skills include diagnosing and managing diseases of the sinuses, larynx (voice box), oral cavity and upper pharynx (mouth and throat) as well as structures of the neck and face, such as the thyroid and parathyroid glands. Facial reconstructive surgery and the repair of facial bone structures are also performed. PHYSICAL THERAPY CLINIC The Physical Therapy Clinic provides rehabilitation, pre- and post-operative physical therapy and therapeutic evaluation for upper and lower extremities. SAME DAY SURGERY This unit offers services to all patients referred by specialty care clinics. Same Day Surgery provides pre-operative teaching and after surgery instructions, post-anesthesia care and discharge teaching. Clinic hours are Monday through Friday, 6 a.m. to 6:30 p.m. For questions, call (580) 558-3400. Additional Patient Care Services OCCUPATIONAL HEALTH CLINIC The Occupational Health Clinic provides services for certain federal and military employees of Ft. Sill, including work-related surveillance, physical qualifying exams, hazardous exposure monitoring and counseling, and limited care for injuries under the federal Office of Workers' Compensation Program. The clinic hours are Monday through Wednesday, 7 a.m to 4 p.m.; Thursday, 7 a.m. to noon; Friday, 7 a.m. to 4 p.m. The Occupational Health Clinic is located in the main hospital on the 3rd floor. Appointments can be made by calling (580) 558-2237. PATHOLOGY Pathology is a full service laboratory that includes specimen collection. All patients should report to Specimen Collection in the outpatient mall for services. Hours of operation are Monday through Friday, 7 a.m. to 4:30 p.m. You can reach the staff at (580) 558-2305. Laboratory orders from in-house providers are submitted electronically, a lab request form must accompany all orders from providers outside of RACH. The laboratory cannot release results to patients. You may obtain them from either the provider or the Patient Administration Division. For assistance call (580) 558-2730. Specimen Collection Located in the clinic mall, this service is a satellite of Pathology. In-house providers submit lab orders electronically; network providers hand write orders and patients bring the request to the lab. Patients must contact the ordering provider for test results. The lab is open Monday through Friday, 7 a.m. to 4:30 p.m. For questions, call (580) 558 2305. PHARMACY The pharmacy provides comprehensive pharmaceutical services to all eligible inpatients and outpatients. There are two facilities that dispense medications to our patients: The Main Pharmacy is located just inside the hospital's outpatient mall entrance and the Pharmissary is located next door to the Commissary. Main Pharmacy The hospital pharmacy is open Monday through Friday, 8 a.m. to 6 p.m.; Saturday, 9 a.m. to 1 p.m. The Main Pharmacy is available to fill all new and refill prescriptions from both RACH and off-post providers. Beneficiaries are encouraged to request refills using either the phone or internet that allow pharmacy staff to fill them during non-peak hours to reduce the wait time. Refills requested by phone or internet can be picked up at window #1 without drawing a pharmacy number. The Main pharmacy also offers patients the option to drop off prescriptions and return in 4 hours. Beneficiaries should select a "D" ticket which will prompt the staff to quickly call them to window #2 and they will be instructed to return to window #1 in 4 hours to pick up their prescription. You can elect to pick up your refills at any time of the day or night by using the Automated Prescription Machine (APM). To use this service you must first register at window #6 in the Main Pharmacy. When you call to refill your prescriptions, select the APM location and your medications will be placed in the APM machine located at the south end of the hospital next to the information desk. A list of medications carried by the pharmacy can be found online by logging onto http://www.rach.sill.amedd.army.mil and clicking on 'Pharmacy' and then 'Pharmacy Drug List.' To speak with a pharmacy representative please call (580) 558-2442/2443 and select option 5. Pharmissary Only prescription refills, faxed or hand- written prescriptions from network providers are dispensed at the Pharmissary. The hours are Monday through Friday, 9 a.m. to 6 p.m. PRESCRIPTION REFILLS RACH patrons may use either the automated phone system or the Internet to request refills. Phone - Dial (580) 558-2442/2443 and follow the directions. Internet - Visit http://www.tricareonline.com and click on 'Refill Prescriptions' link. Internet - Visit http://www.rach.sill.amedd.army.mil/pharmacy.php and click on 'Online Refills' button. Refills called in by 7 a.m. will be ready the same day after 1 p.m. Refills called in after 7 a.m. will be ready after 1 p.m. the next day. Designate which pharmacy you would like to use to pick-up your prescriptions. Home Delivery Program The Home Delivery program sometimes called TRICARE Mail Order Pharmacy (TMOP) is a good choice for your prescriptions. The program is designed for medications you are taking on a regular basis. With home delivery, you can receive up to a 90 day supply of medication for the same cost as a 30 day supply at a retail pharmacy. Active Duty cost share is $0. All other beneficiaries, for up to a 90 day supply, pay as follows: You can fill your first prescription by mail. You can order refills by mail, phone, fax or online. You can avoid driving to a retail pharmacy and waiting in line. Your prescriptions will be mailed to your home. This includes any U.S. postal address, APO/FPO, or temporary address with no shipping and handling fees. Register for Home Delivery online at http://www.express-scripts.com or call (877) 363-1303. ARMY PUBLIC HEALTH NURSING Army Public Health Nursing is dedicated to sustaining military readiness through health promotion, wellness, and disease prevention activities for the Fort Sill community. We offer a myriad of services to include but not limited to HIV/STI education and intervention, CYSS health consultation, Tuberculosis surveillance and Latent Tuberculosis Clinic, Tobacco Cessation, and various education classes such as Self-Care and women's health. Army Public Health Nursing Preventive Medicine is located on Ring Gold Road. Hours are Monday through Friday, 7:30 a.m. to 4:30 p.m. To schedule appointments and classes or for any questions please contact Army Public Health Nursing at (580) 442-2061. RADIOLOGY The Radiology Department takes pride in offering a full range of diagnostic imaging. These services are available to our patients by referral from both in-house providers and downtown providers. The department serves as a training site for radiology technology students and we feature the latest in imaging technology. Our services include diagnostic radiology, mammography, Computerized Tomography (CT), Magnetic Resonance Imaging (MRI), Ultrasound and Nuclear Medicine. Some appointments will require you to schedule in person so you can receive the proper preparation for your exam. If the PCM is at RACH or FMH contact our staff at (580) 558 2780 for questions or to schedule an appointment. Reynolds offers a full range of mammography services and is accredited by the American College of Radiology and certified by the Food and Drug Administration. We have an all-female staff of mammographers, who hold advanced certification in mammography. RACH has the newest digital mammography technology available; using computers to produce images that are displayed on high resolution monitors. We offer a wide range of breast biopsy services, using a state-of-the-art digital stereotactic biopsy system. This allows us to perform minimally invasive breast biopsies in a comfortable environment. Patients are able to self-refer for a screening mammogram if the primary care manager is at RACH. RESPIRATORY THERAPY/ PULMONARY FUNCTION TESTING The Respiratory Therapy Department provides several types of Pulmonary Function Testing such as spirometry, lung volumes, diffusion, exercise challenge, methacholine challenge, impulse oscillimetry, home oxygen evaluations and arterial gas sampling. Please call (580) 558 2642 for assistance. Behavioral Health Services CHAPLAIN SERVICES The Chaplain and Pastoral Care are available 24 hours a day to in-patients, Soldiers and their loved ones. Protestant worship is held in the Chapel on Tuesday at 11:45 a.m. The Chapel and adjoining Meditation Room are open 24 hours a day. The Chapel is located near the south entrance of the hospital. All denominations are welcome. The Chaplain's office may be reached by calling (580) 558 2615/2616. After 5:30 p.m. the chaplain on call can be reached by calling the Staff Duty Officer at (580) 558 2800. COMMUNITY BEHAVIORAL HEALTH SERVICES (CBHS) CBHS provides services to Active Duty Service Members primarily. This includes members of the National Guard and Reserve who are in an active status. All beneficiaries are provided urgent triage and crisis intervention with appropriate treatment planning and referrals during duty hours. Soldiers can self-refer to this clinic. Various assessment and psycho-therapeutic interventions are available. Psychiatric Sick Call Hours are Monday through Friday, 7:30 to 11 a.m. All walk-ins that occur beyond 11 a.m. to 4 p.m. are triaged by the on-call clinician. Any Service Member experiencing need for emergency services after hours, or during the weekend hours, should report to RACH ER or the nearest local emergency room. CBHS is located in Building 2442 and can be reached at (580) 442-4832/4833/4351. The Soldier Readiness and Resiliency Center/Testing Center is located at Building 3445 and can be reached at (580) 442-4678. SOCIAL WORK SERVICES (SWS) Social Work Services (SWS) offers services that promote healthy, resilient families, individuals and relationships focusing on prevention, early treatment and post-traumatic growth. Soldiers, Retirees and Family Members can self-refer to this clinic for individual, marital and family counseling. Anger management, combat stress classes, stress management classes, resiliency training for Service Members and their families, relationship enhancement, conflict resolution and parenting skill groups are available on a limited basis. SWS also provides comprehensive services for individuals and families referred to the Army Family Advocacy Program. Contact SWS to find out the next start date for the group of your choice or to find out which service fits your need. SWS is located at Building 3161 and is open Monday through Friday, 7:30 a.m. to 4:30 p.m. For information, call (580) 442-2836/8748/8750. OUTREACH PROGRAM The Outreach Program is in Building 2873. The program is available to Soldiers and their families. The staff assists Soldiers preparing to deploy, those who have deployed and the families of deployed Soldiers. In addition to screening Soldiers and directing the level of care, they perform crisis intervention and assist the Chaplain at reunion briefings. Inpatient Services CLINICAL DIETETICS BRANCH Clinical Dieticians provide nutritional screenings and medical nutrition therapy for inpatients at high or moderate nutrition risk. The phone number is (580) 558 2823. MATERNAL CHILD UNIT The Maternal Child Unit (MCU) offers 16 Labor, Delivery, Recovery and Postpartum (LDRP) Suites. This style of obstetrical care allows the patient to labor, deliver, recover and room-in, all in the same area. Prenatal classes are offered the first Saturday of each month from 9 a.m. to 1 p.m. Prenatal Class I: An overview of the childbirth process to include anatomy, physiology, plan of care and phases of labor. Prenatal Class II: Obstetrician/Anesthesia Services. Prenatal Class III: Helps prepare moms for early stages of labor. Couples bring pillows and a blanket to do practical exercises on the floor. Prenatal Class IV: Postpartum, length of stay, self care for mom and baby and infant security. The Breastfeeding Class is offered the third Thursday of the month, 1 to 2 p.m. immediately following prenatal classes for those who would like to attend. Sibling classes are by appointment only; call MCU at (580) 558-2662. Reynolds is proud to be the only hospital in the Lawton area with nurse-midwife services.Midwifery is a health care profession where providers give prenatal care to expecting mothers, attend the birth of the infant and provide postpartum care to the mother and her infant. RACH is committed to providing outstanding, family-centered and safe maternity care for all of our patients. Our providers understand the unique needs of military families, especially in today's climate of increasing deployments. We have highly trained, dedicated professional staff who are "Proudly Delivering America's Future." The PCU is a medical/surgical unit that provides care to all ages. Visitors should check in at the nurse's station. The PCU is located on the second floor of the hospital. Fathers, or support persons, may stay 24 hours. PROGRESSIVE CARE UNIT (PCU) The PCU is a medical surgical unit that provides care to all ages. Visitors should check in at the nurse's station. The PCU is located on the second floor of the hospital. Please call (580) 558 2605 for additional information. Patient Support Services AMERICAN RED CROSS Volunteers are important members of the RACH Team. They serve in various areas of both the hospital and dental clinics. Individuals interested in volunteering should contact the American Red Cross at (580) 558 2132/3052 or visit their office on the third floor of the hospital. Some prerequisites for volunteering include a three hour orientation to the Red Cross and hospital, a health screening and completion of privacy training. In addition to an adult volunteer program, VolunTEENs (ages 14 to 18 years old) provide service at RACH and other areas on post during the summer months. BENEFICIARY COUNSELING ASSISTANCE COORDINATORS (BCAC) AND DEBT COLLECTION SERVICES BCAC's support beneficiaries by providing information and assisting with enrollment, claims processing, eligibility queries and travel entitlement. They provide TRICARE briefings and serve as the point of contact for debt collection cases. Staff is located at the south entrance next to the TRICARE Service Center. Call (580) 558 2483/3161 for assistance. BIRTH CERTIFICATES To obtain an official copy of a birth certificate, you must submit a completed Birth Certificate Request form. Forms are available at the Patient Administration Division. DINING FACILITY The Dining Facility is located at the south entrance of the hospital on the first floor. MARKETING/EDUCATION The Marketing/Education Office is actively involved with and supports various groups and programs in the area. We strive to keep Fort Sill and the general public educated and updated on the TRICARE Benefit and RACH services. Information briefs are held quarterly at the hospital and are open to the public. Dates and times of briefs are posted in The Cannoneer newspaper and on the RACH Internet. For your convenience, any unit, group or organization is welcome to schedule a brief during duty hours or after hours. MEDICAL CORRESPONDENCE/ RELEASE OF INFORMATION This office assists patients who need copies of their medical information. Patients must present an ID card and complete the release form. If the requestor is someone other than the patient, a medical power of attorney will be needed. This office is located with Inpatient/Outpatient Records in the basement on the north side of the hospital. Contact staff at (580) 558 2103 or fax to (580) 558 2756. INTEGRATED DISABILITY EVALUATION SYSTEM (IDES/MEDICAL BOARDS) IDES helps the Department of Defense (DoD) determine whether wounded, ill or injured Service members are able to continue to serve. IDES quickly returns to duty those who are. For those who are not, the IDES process determines the disability ratings they will receive from DoD and the Department of Veterans Affairs (VA). DoD and VA joined together to create IDES to integrate processes DoD and VA formerly executed separately. This new, streamlined system mproves America's delivery of disability services and benefits to Service members, Veterans and their families. Your primary care doctor recommends possible medical board evaluation. The Soldier is referred to and evaluated by the Medical Board Physician who initiates a permanent profile as necessary. After the profile is issued, the Soldier reports to Patient Affairs, Medical Board Section where a briefing and appointments for Phase I and Phase II of a physical exam are scheduled. A medical board packet is prepared by the Soldier's command and submitted to the Medical Board Section within seven days. The Soldier's packet is forwarded to the Physical Evaluation Board for a final decision. Once results are received, the Soldier is counseled on his/her elective options. NUTRITION DIVISION CLASSES Weight Management, Heart Health and Cholesterol classes for Active Duty Service Members, Family Members and Retirees are available. Diabetes Nutrition Education classes are held the first and third Tuesday of each month. Patients need to bring both a glucometer and a list of current medications to the class. Bariatric Support Group classes are provided upon referral from the General Surgery Clinic. Dates and times of classes may vary. OPERATION WHEELS "Operation Wheels" is a unique service of the Armed Services YMCA and offers FREE TRANSPORTATION for necessary services such as medical appointments, dental appointments, legal appointments and commissary trips. Reservations must be made 24 hours in advance. Junior enlisted Soldiers and their Family Members have priority. Call (580) 355 5520 for more information. OUTPATIENT RECORDS Medical records are no longer hand-carried by Soldiers when making a permanent change in duty station. Records will be mailed to the new Military Treatment Facility once the Soldier brings a copy of his/her orders to Outpatient Records. Only Soldiers on recruitment orders, going to a remote location or overseas are authorize to hand-carry records. If dependents are 18 years or older, access to their medical information must be authorized through a signed notification, but hand-carried by them only. PATIENT ADVOCATE OFFICE Patient Advocates assist patients and Family Members resolve their health care concerns, by serving as a liaison between the patient and staff. PATIENT AFFAIRS BRANCH Patient Affairs is responsible for processing all medical boards for Soldiers whose military career has been interrupted by an injury or illness. They arrange fit-for-duty physicals for all Guard Soldiers as well as line of duty preparation for Soldiers who acquire an injury while on Active Duty. Soldiers placed on Temporary Disability Retirement List (TDRL) are scheduled for appointments and processed through Patient Affairs. WOMEN, INFANT AND CHILDREN (WIC) PROGRAM The WIC Program is a nutrition and education program based on income level for pregnant and postpartum women, infants and children. Frequently Asked Questions FREQUENTLY ASKED QUESTIONS I am New To Fort Sill—How do I Obtain Medical Care? There are two things you must do before obtaining medical care. First, you must update your status in DEERS (Defense Enrollment Eligibility Recording System). Second, you must enroll in TRICARE Prime or transfer your current enrollment to Fort Sill. TRICARE Prime enrollment is mandatory for Active Duty Soldiers and voluntary for Family Members and retirees. Soldiers must take action to enroll within one week of unit assignment. Enroll at the TRICARE Service Center or online at http://www.humana-military.com. What is a Primary Care Manager (PCM)? After you complete TRICARE enrollment, you will receive a letter notifying you of your assigned Primary Care Manager (PCM). A PCM is a health care provider who provides for your health care needs and coordinates specialty care as needed. If you do not receive notification of your assigned PCM within 14 days after enrolling, please contact Humana Military at (800) 444 5445 or visit the TRICARE Service Center in the Hospital. How do I get an Appointment? You may schedule an appointment with your PCM by calling (580) 558 2000. One of the booking staff will assist you with your appointment needs Monday through Friday, 7 a.m. to 4 p.m. Over 80 percent of our primary care appointments are accessible online. For your convenience, you can access and schedule an appointment with your PCM up to 24 hours in advance. We encourage you to register with TRICARE online and make your appointments at http://www.tricareonline.com. What do I do if I Have a Medical Problem After Hours? How do I Reach my Primary Care Manager? If you have an emergent need, please go to the nearest emergency room to receive care. If emergent care is received at a civilian emergency room, please notify the Referral Management Office of this emergency care within 24 hours so authorization for payment can be made. Do not seek authorization until after you have received emergency care. For non-emergent (urgent) medical needs after hours, you may contact the on-call PCM by calling the hospital's answering service at (580) 558 2000, or toll free at (866) 207 7603. Please select the prompt after hearing the message, "To reach the on-call physician for out of area care authorization," press option 3. The on-call PCM may give you advice over the phone that allows you to wait until the next day for a regular clinic appointment. If your need is more urgent they will direct you to be seen at the RACH Emergency Room. What do I do if I am Traveling out of the Area and need Authorization for Medical Care? If you have an emergent need, please go to the nearest emergency room to receive care. Failure to do so could cause your claim to be denied and you could be held liable for all costs. Do not seek authorization until after you have received emergency care. If you have a medical problem that is non-emergent you must have an authorization prior to seeking/receiving health care in order to avoid point of service claims. You may speak to one of the nurses in the Referral Management Office during normal business hours for direction of care by calling toll free at (866) 207 7603, option 4 or (580) 558 2000, option 4. They can give you advice on whether or not you should see a provider while you are traveling or wait until you return home. If urgent care is approved they will enter a referral that will be sent to Humana Military for authorization. If you need assistance with finding a network provider for this approved urgent care, please call Humana (TRICARE) at (800) 444 5445. For an after-hours authorization, please contact the on-call PCM by using the after hours process described in Question Four, utilizing the toll free number. Who do I go to with Questions or Concerns about my Provider or Health Care? For health care or customer service concerns ask for the NCOIC or Office Manager of your clinic. In most cases they will be able to assist you and provide you with answers to your questions. If you need additional assistance, see one of our Patient Advocates, located on the first floor of the hospital, or call (580) 558 2389/2390. How do I Receive Pregnancy Testing? If you think that you are pregnant, you can walk-in to the OB/GYN clinic to request a pregnancy test. Walk-in hours are Monday through Friday, 7:30 a.m. to noon and 1 to 4 p.m. A nurse from the OB Clinic will call you within 24 hours with your results. Do I Need to Bring my Insurance Information to my Appointments? Yes. On your first visit you will be asked to complete a Record of Other Health Insurance, DD Form 2569. Completing the form will take approximately two to three minutes. After the form has been processed, you will receive a small insurance card. Please bring this card to each visit. The card's information will need to be updated annually. The third party collection program is mandated from the Surgeon General and we appreciate your time and assistance in helping us meet this requirement. How do I obtain Medication Refills? There are two easy ways to refill your medications: By phone or online. Call (580) 558-2442/2443. Prescription refills for either pharmacy can be phoned in by dialing (580) 558-2442/2443. Refills called in by 7 a.m. will be ready the same day after 1 p.m. Refills called in after 7 a.m. will be ready after 1 p.m. the next day. Active Duty Soldiers Sick Call hours are Monday through Friday, 7:30 to 9 a.m. For questions, or to schedule an appointment, call (580) 558 2710.
