Monday through Friday, 7:15 a.m. to 4:30 p.m. Satellite pharmacy: Monday,Wednesday, Friday, 7:30 a.m. to 4:30 p.m.; Tuesday, Thursday, 8:30 a.m.to 4:30 p.m.; the clinic is closed the third Friday of each month for training The 45th Medical Clinic offers basic services in family practice, pediatrics, internal medicine, physical therapy and orthopedics. It provides medical, dental and optometry services to activeduty and retired military personnel and their families, with more than 3,200 active-duty and 35,000 nonactive beneficiaries. A staff of 140 health care personnel make it their mission to help maintain peak readiness by ensuring a fit force, a healthy community and medics ready to support global operations.

Patients requiring general care at the base clinic should call the appointment phone line to schedule treatment. Emergency medical care is not available at Patrick Air Force Base. Call 911 or go to the nearest emergency room with lifethreatening injuries or illnesses.

Holmes Regional Medical Center features 514 beds and more than 500 highly trained medical staff members. The facility is the only stateaccredited Level II trauma center in Brevard County, while the Heart Center at Holmes performs more cardiac procedures than all other hospitals in Brevard and provides patients with all-private rooms. In addition, Holmes was the first hospital in Brevard to offer state-of-the-art robotics-assisted surgery for urology, prostate and gynecological conditions and is the only hospital in Brevard to offer anterior hip replacement surgery, reverse shoulder surgery and interventional neuroradiology procedures for advanced stroke treatment. TRICARE
The primary objectives of this health care program for active duty and retirees of the uniformed services, their families and survivors are to optimize the delivery of health care services and attain the highest level of patient satisfaction. In addition, the coverage is completely portable so it moves with you when you relocate.

Follow these simple steps to ensure you have no break in coverage when you move: Continue your TRICARE Prime enrollment option.

Immediately update your personal information in the Defense Enrollment Eligibility Reporting System when you arrive at your new location. Select a new primary care manager when moving within the same region or submit a Prime enrollment form within 30 days if you're moving to a new region.

Location will determine your new Prime option. If you move to a new Prime service area, transfer your enrollment by submitting a TRICARE Prime Enrollment and PCM Change Form to your regional contractor. Be sure to include all family members who wish to use Prime on the form.

Your TRICARE Prime options include TRICARE Prime and TRICARE Prime Remote in the United States, and TRICARE Prime Overseas and TRICARE Global Remote Overseas in regions outside of the United States. TRICARE Standard and Extra are available to family members only. Active-duty service members are required to enroll in one of these Prime options. The Standard and Extra options, on the other hand, are available throughout the United States and enrollment is not required. Any active-duty family member who is registered in DEERS may use these programs by seeing any network or non-network provider that is authorized by TRICARE.

If you're already using TRICARE Standard and Extra, moving is easy. When you arrive at your new location, immediately update your personal information in DEERS. Then find TRICARE-authorized providers in your new area.

Here are a few things to remember about using TRICARE Standard and Extra in a new region: In the U.S., visit your new regional contractor's website for a list of providers. If you use network providers, your copayments will be lower.

If you move to a new region, you'll have a new claims address for submitting your TRICARE claims.

Learn your new region's prior authorization requirements as these requirements may differ by region.

EMERGENCY SERVICES
AND URGENT CARE CENTERS
In the event of an emergency, that would result in loss of life, limb or sight, call 911 for ambulance service or rush to the nearest offbase emergency room. Use urgent care instead of emergency care for an injury or illness that requires professional attention within 24 hours but is not a true life-threatening emergency. Contact your PCM before or within one day of being seen in an urgent care center or emergency room.
