E komo mai—welcome to Naval Health Clinic Hawaii (NHCH) where your health and readiness are our focus. NHCH is comprised of two large clinics, Makalapa at Pearl Harbor Naval Base and Kaneohe Bay at MCB Hawaii. Other smaller clinics are located at Camp H.M. Smith, Wahiawa and Barking Sands on the island of Kauai. Of note, the headquarters for NHCH is not co-located with the clinics, but resides in Building 1750 at 480 Central Ave. adjacent to the Pearl Harbor Naval Shipyard. Preventive Medicine, Industrial Hygiene and Occupational Health Services are provided at this site, which is also known as the Shipyard Clinic. Welcoming Center
Medical care at NHCH begins with the Welcoming Center, where active duty members, family members and retired PRIME beneficiaries begin a one-stop check-in process to enroll to NHCH for care. Trained staff assist with TRICARE enrollment, Medical Record updating, Primary Care Manager (PCM) assignment and initial appointments for any needed medical care. Active duty members also complete the Periodic Health Assessment (PHA), part of the Individual Medical Readiness process. TRICARE Representatives and Health Benefits Advisors are available to answer TRICARE and enrollment questions. During the check-in process, information about several resources, including TRICARE Online (http://www.tricareonline.com, a website that allows you to make medical appointments), Exceptional Family Member Program and Advance Directives will be provided.

Hours of operation and telephone number: Makalapa Clinic: Monday through Friday, 7:30 a.m. to 5 p.m.; Saturday, 8 a.m. to 4 p.m.; and Sunday, 8 a.m. to noon.; Telephone (808) 473-2444, ext. 501. Kaneohe Bay Clinic: Monday through Friday, 7:30 a.m. to 4 p.m. Telephone: (808) 257-3365, ext. 251/250.

Please bring ID cards, Social Security numbers (for all enrolling), medical records, dental records (if active duty) and immunization records when you check-in. If you have your medical record, we will request it be sent from your last provider. You can still complete the check-in process. If you have never had a military health care system medical record, we will make one for you. For further information about the Welcoming Center, please call the Welcoming Center at the site you wish to enroll.

Clinic Locations, Hours of Operation and Services
For life threatening conditions, dial 911 or go to the nearest emergency room.

Medical Appointments: There are several ways to make an appointment with your Primary Care Manager (PCM):
Call NHCH Central Appointments at (808) 473-0247.
Log on to http://www.tricareonline.com and click the link under "My Health."
Call your PCM's clinic directly.

NHCH After-Hours Provider Advice Line: Lets you speak with a medical provider when the clinics are closed. This service is accessed by calling (808) 473-0247, ext. 3. You will speak with an answering service who will contact an after-hours provider.

A provider will return your call within the hour. Please note, the provider has no access to labs, X-rays, appointments or medical records.

Makalapa Clinic: Located on Pearl Harbor Naval Base just inside the Makalapa Gate, in Building 1407, off Makalapa Road. Hours of operation are Monday through Friday and Sunday, 8 a.m. to noon, (for all NHCH enrollees).
*Closed on Thanksgiving Day, Christmas Day and New Year's Day.

Services provided include Family Practice, Military Medicine, Pediatrics, Internal Medicine, Dermatology, Gynecology, Dental, Immunizations, Laboratory, Optometry, Pharmacy, Preventive Medicine and Radiology. Physical Therapy and Sports Medicine services are located at the Makalapa Annex (across the parking lot from the main clinic, beside the Welcoming Center). Mental Health and Substance Abuse and Rehabilitation Program (SARP) services are provided at Building 1, located on Russell Ave. across from the Shipyard Training Facility.

Kaneohe Bay BHC: located on MCB Hawaii in Building 3089, off D Street.
Hours of operation are Monday through Friday, 7:30 a.m. to 4 p.m.

Services provided include Family Practice, Military Medicine, Pediatrics, Optometry, Pharmacy, Laboratory, Radiology, Preventive Medicine, Occupational Health, Aviation Medicine, Dermatology, Mental Health and Deployment Health. Physical Therapy and Sports Medicine services are provided in Building 221.
