Evans Army Community Hospital (EACH)
Evans Army Community Hospital is located in Building 7500 off Titus Boulevard, near the golf course. The five-story
hospital has a 78-bed capacity while an adjoining, two-story clinic building contains 400 examination and treatment rooms.
A common area atrium connects the two sections of Evans hospital. This area includes the main entrance, medical library,
chapel and pharmacy.
Adjacent to the hospital is a 153,350 square-foot, three-story Soldier Family Care Clinic. This houses family practice,
pediatrics, a 24-chair dental clinic, physical therapy, occupational therapy, podiatry, orthopedics, chiropractic, social work
services and family advocacy, outpatient records, ancillary radiology, laboratory services and pharmacy services.
The hospital also has four external Family Medicine Clinics on Fort Carson and one in north-central Colorado Springs.
Military Families can enroll in TRICARE Prime by
visiting EACH's TRICARE Service Center located in
room 1011, first floor of the main hospital near the west
entrance. TRICARE Prime provides patients with the
best means of access to health care at EACH. Family
members must be enrolled in TRICARE Prime or their
care may be limited. TRICARE Standard patients are
seen on a space-available basis only.
The Patient Administration Division of EACH helps
Soldiers and Families order medical records from other
military facilities and civilian hospitals, copy records,
process birth certificates, conduct Defense Enrollment
Eligibility Reporting System checks, initiate medical
boards; and provides notary services and processes
requests for release of medical information. For more

All patients older than 10 years of age must present
an identification card for treatment at EACH. A parent or
guardian must accompany children under 18.
All military Family members must be enrolled in
DEERS prior to receiving nonemergency medical care.
Records for Family members are maintained at the
hospital and are delivered to the clinic for appointments.
For those who do not have records, the Outpatient
Records Section will create them. Questions concerning
records may be answered by calling 526-7298.
How to make an appointment
TRICARE beneficiaries can make, cancel and reschedule
appointments or talk to an advice nurse by calling the Pikes Peak
Regional Access to Care line, 457-CARE (2273) or by accessing
TRICARE Online at http://www.tricareonline.com.
Patient Advocate Office
The Patient Advocate Office serves patients who have questions,
problems or concerns about medical care or medical staff. It also
assists beneficiaries with resolving medical debt collection and
issues with TRICARE claims. For more information, call 526-7225,
for billing issues call 526-7532. After duty hours, contact the
administrative officer of the day at 526-7001/7002.
EMERGENCY DEPART MENT
Patients requiring emergency care should be taken directly to the
hospital's Emergency Department. Emergency services are available
24 hours a day. The telephone number is 526-7111. Patients are
seen on a medical priority basis, based on the seriousness of the
injury or illness, not necessarily in the order of arrival.
AMBULANCE SERVICE
For ambulance service on Fort Carson, call 911 from any civilian
or military phone. The Fort Carson ambulance service will respond
to on-post emergencies only. Civilian ambulances will respond to
off-post emergencies.
Pharmacy Services
Prescribed medications and refills are dispensed without charge
to eligible military beneficiaries. The main hospital outpatient pharmacy
is open Monday through Thursday 8 a.m. to 6 p.m., Friday 8
a.m. to 5 p.m. and Saturday 8 a.m. to 4 p.m. Federal holiday hours are
8 a.m. to 4 p.m. and closed on Thanksgiving and Christmas Day.
The Commissary Satellite Pharmacy, located next to the post
commissary, is open from 9 a.m. to 5 p.m., Monday through Friday
and closed weekends and Federal holidays.
Full-service pharmacies are also located within the Family
Medicine Clinics, including Warrior, DiRaimondo (TMC 9), Robinson
(TMC 10), Premier Army Health Clinic and in the Soldier Family Care
Center for patients receiving care at these clinics. Operating hours

Prescription refills should be ordered in advance either by phone
at 524-4081 or 888-745-6427, or by using the online ordering
system: http://www.evans.amedd.army.mil/scripts/mgwms32.dll?MGWLPN=DOD&WEBAPP=PHARMACYENTRY. Refill prescriptions
ordered before 8 a.m. Sunday-Thursday are ready within two
duty days and generally after noon. Refills can be picked up at either
the main Evans hospital outpatient pharmacy, located near the
dining facility, or may also be picked up at the Commissary Satellite
Pharmacy, located in Building 1525.
Mountain Post Wellness Center
This center offers a unique team created to meet the resiliency
needs of Soldiers, Families, retirees and the civilian workforce. The
Mountain Post Wellness Center combines the expertise of Evans
Army Community Hospital, Army Community Service, Family Life
Chaplain, Public Health Command and U.S. Army Garrison. The
center uses a holistic approach to address mind, body and spirit,
meeting the vision of comprehensive Soldier fitness. Healthy soldiers,
families, retirees and civilian workforce will build Resiliency.
Services include: Personal wellness profile, exercise prescription,
weight management to include MetaCheck, cholesterol and blood
pressure management, stress management techniques with relaxation
chair and Migun bed, Civilian Wellness Program and active-duty
pregnancy/postpartum education.
The Mountain Post Wellness Center's mission is to embrace the
military community through a world-class health experience focused
on fitness of mind, body and spiritual wellness. The center and its
staff embrace their vision as the destination of choice leading to
healthy lifestyle changes, performance enhancement and resiliency

Army Substance Abuse Program
Fort Carson is a drug-free community. Soldiers, civilian employees,
retirees and their Families work to prevent the abuse of alcohol,
the use of illegal drugs and abuse of prescription drugs.
The Fort Carson ASAP mission is to strengthen the overall fitness
and effectiveness of the Army's total workforce and to enhance the
combat readiness of its Soldiers through its care.
The alcohol and drug control officer of the Army Substance Abuse
Program at Fort Carson is the point of contact for administration of
nonclinical programs and services. The ASAP team office is located
at 1675 Ellis St., Building 1217. Call 526-2181 for more information.
Fort Carson's clinical ASAP is located at 1636 Elwell St. The
ASAP clinic staff assesses, evaluates and treats active-duty Soldiers,
refers for detoxification and residential treatment and refers civilians
for treatment as necessary. Call the ASAP clinic at 526-2862.
U.S. ARMY DENTAL ACTIVITY
Complete dental care is available for all active-duty Soldiers.
Fort Carson has state-of-the-art dental facilities. Commanders and
first sergeants will direct Soldiers to the appropriate facility. For more

Due to limited resources and an emphasis on active-duty dental
readiness, there is no space available for Family members. Family
members of active-duty Soldiers may apply to be enrolled in the
TRICARE Dental Program. Services covered under TDP include
endodontics, periodontics, oral surgery, prosthodontics, orthodontics,
crowns and other cast restorations in addition to preventive, diagnostic,
emergency, anesthesia and restorative services. Enrollment forms

After-Hours Emergency Dental Care
After-hours dental emergencies (bleeding, swelling or trauma)
should report to Evans Army Community Hospital emergency room.
FORT CARSON VETERINARY TREAT MENT FACILITY
To promote the health and well being of animals owned by
military personnel, the Fort Carson Veterinary Treatment Facility
sees pets on an appointment-only basis. For more information or to
schedule an appointment, call 526-3803. The VTF is located at 1535
Sheridan Ave. in Building 6190.
Anyone who owns pets and lives on post, must abide by Fort
Carson Regulation 40-5 and the Fort Carson Family Housing Guide:
• Register pets with the VTF within 10 days of arrival, or after
acquiring a new pet. Fort Carson residents are limited to two
pets in post quarters.
• Have all puppies and kittens vaccinated with an initial series of
shots beginning at six-eight weeks of age, every three to four
weeks until 16 weeks of age and annually thereafter. Have all
pets vaccinated for rabies at 16 weeks of age and annually
thereafter. The rabies identification tag must be placed on the
animal's collar.
• Have pets microchipped. This involves placing a microchip, the
size of a grain of rice, underneath the skin in the pet's shoulder
area. This is a means of permanent identification and is
mandatory for pets living on Fort Carson.
• Provide adequate food, water and shelter for animals; remove
pet waste daily.
• Animals must be under proper control at all times and cannot
roam freely. Animals will not be left unaccompanied in vehicles.
• For those who live off post and within the city limits of Colorado
Springs, their dogs and cats must be vaccinated against rabies
annually. Dogs must be licensed within 30 days of arrival and
annually thereafter.

The clinic hours of operation are Monday through Friday from
7:30 a.m. to 3:30 p.m. It is closed holidays and the last working day
of the month. These hours are subject to change on very short
notice, based on mission requirements.
Warrior Transition Battalion (WTB)
In the summer of 2007, the Army Medical Action Plan was created
to help develop solutions to improve processes and procedures
for providing care to ill and injured Soldiers at Army and VA medical
treatment facilities. In today's Army, wounded, ill and injured Soldiers
are more commonly known as warriors in transition.
The mission of the Fort Carson WTB is to provide command and
control, primary care and case management for warriors in transition
in order to establish conditions for their healing and to promote a
timely return to the force or transition as a veteran to civilian life.
Every wounded warrior in the WTB receives a detailed comprehensive
care plan detailing a plan of action to medically heal
the Soldier. This plan in turn is
an integral part of a detailed
comprehensive transition plan
which addresses medical care,
spiritual welfare, behavioral health
counseling, family counseling,
education and meaningful,
productive employment.
The goal is to transition a
healed and trained Soldier back
into the Army or to transition
a healed, educated veteran to
productive civilian life.
