COMMISSARY
The commissary is located at 17601 25th St. The facility features 12 checkout registers, a full service deli, bakery, meat and produce department and over 13,699 line items stocked in the grocery department.

The commissary also offers early bird hours for those with 15 items or less Tuesday through Thursday, 8:30 to 9 a.m. If a holiday falls on a Monday, the commissary will be open the following Tuesday. Handicapped people are authorized to enter the commissary one-half hour early to shop. Military people in uniform are authorized to go to the head of either line with 15 items or less between the hours of 11 a.m. and 1 p.m. For more information, call the Commissary at (530) 634-2422.

LEGAL SERVICES
The office of the Staff Judge Advocate, at 17801 Warren Shingle Blvd., provides several important services for newcomers to Beale, including legal assistance (wills and powers of attorney, notary services and legal advice), assistance in filing claims for loss or damage to personal property (household goods claims) and state and federal income tax preparation. Legal Assistance: The office provides legal assistance to active duty, their dependents and retirees. Licensed attorneys are available for advice and consultation on personal civil legal matters (land lord tenant issues, divorce, custody, consumer issues, leases, residency and many other issues). The legal office also offers assistance in the preparation of legal documents and instruments such as wills, bills-of-sale and powers-of-attorney. In addition, there are helpful preventive law brochures to provide guidance on domestic relations, wills, consumer law and other legal topics.

Walk-in legal assistance is available Monday and Tuesday, noon to 2 p.m.; Wednesday, 1 to 3 p.m.; Thursday, 10 a.m. to noon; and Friday, 11 a.m. to noon. Copies of the various brochures can be obtained by visiting the legal office. Notary services are offered Monday through Friday, 7:30 a.m. to 4:30 p.m.

Claims: Claims for lost or damaged household goods, privately owned vehicles shipped at government expense and theft or vandalism of private property located on the installation may be filed with the claims office by calling (530) 634-3294/3295. The claims office conducts a claims briefing on Thursday at 10 a.m. Forms and instructions necessary for filing a claim will be provided at the briefing.

Two important things to keep in mind for household goods claims: noting the damage on the DD Form 1840/1840R (pink forms) and turning in the DD Form 1840/1840R to the legal office within 70 days of delivery.

First, it is important to note the damages or loss on the DD Form 1840 at the time of delivery and unpacking. If loss or damage is discovered at a later date, annotate the specified damage on the reverse side of the DD Form 1840 (DD Form 1840R) and bring all copies of the DD Form 1840 and 1840R to the claims office within 70 days from the date of delivery.

Failure to note damage/loss or to deliver the forms within 70 days from the date of delivery may have adverse effects on your claim. Although claimants have two years from the date of delivery to file a claim for loss or damage, it is not recommended to wait until the statute of limitations are about to expire to File a claim.

Income tax preparation: The tax center, located at 6008 B St., provides free state and federal tax return preparation and Filing. This service is normally offered January through April 15, but is expanded for returning deploying members and their families. For more information, contact the tax center at (530) 634-5536.

EMPLOYMENT OPPORTUNITIES
The Non-Appropriated Fund Human Resources Office, located at 17855 Warren Shingle, Suite 179, accepts applications for employment for off-duty military and family members. Applications are accepted for a variety of positions.

AIRMAN & FAMILY READINESS CENTER - 9TH FORCE SUPPORT SQUADRON Military life can be challenging for service members and their families. The Airman & Family Readiness Center helps individuals and families prepare for those challenges with workshops, counseling services, special events, self-help books, reference material and computer programs. If you're unsure of where to turn, the A&FRC should be your first stop. The A&FRC is located at 17800 13th St., on the corner of 13th and B Streets. PERSONAL FINANCIAL
MANAGEMENT PROGRAM
Many people cite financial problems as their primary cause of stress. The Personal Financial Readiness program can help Airmen and their families manage their finances more effectively. Whether you're struggling to make ends meet or are planning a major purchase such as a vehicle or home, PFR can help with classes and individual consultations.

The A&FRC also offers classes in managing credit, banking, car buying, home buying, investing and budgeting. The Bundles for Babies workshop is held with support from the Air Force Aid Society to help expectant parents prepare financially for their new arrival.

AIR FORCE AID SOCIETY
The Air Force Aid Society is the official charity of the Air Force and is located in the A&FRC. AFAS provides special Community Enhancement Programs (Child Care for PCS, Give Parents a Break, Car Care Because We Care, and more). AFAS can also provide interest-free loans or grants to help with essential needs when an Airman is dealing with an unforeseen emergency. Basic living expenses, emergency travel, and vehicle repairs are some categories of assistance that AFAS might be able to help with. Each case is evaluated individually on its own merits.

RELOCATION ASSISTANCE
The Relocation Assistance program educates Airmen and their families to help alleviate some of the stress associated with a PCS move. Personal consultation and workshops such as Smooth Move and the Overseas PCS Briefing help members limit the amount of out of pocket expenditures incurred during a PCS move.

Information is provided to newcomers at the Wing Right Start/Information Fair to assist Airmen in getting settled into their new work center and community. Happy Landings is a "Welcome to Beale" event designed to assist newly arrived spouses in quickly developing their connections at Beale AFB and with the local community. Paid childcare is available; morning refreshments and lunch are also provided.

Members are encouraged to take advantage of the AFAS community enhancement initiative, Child Care for PCS, which offers 20 hours of childcare per child for members arriving at or departing from Beale.

LOAN LOCKER
For those who recently arrived at Beale or who are getting ready to move to their next duty station, the Loan Locker should be one of their first stops. Whether living in temporary lodging or left with an empty house after the movers have completed packing, the Loan Locker can provide essentials for daily living. Located in the A&FRC, the Loan Locker offers dishes, pots and pans, irons and ironing boards, port-a-cribs, booster seats, sleeping mats, toaster ovens, crock pots, TV-DVD players, and more. Items can be checked out for 30 days at a time, free of charge.

SCHOOL LIAISON OFFICER
The School Liaison Officer provides up-to-date educational resources and information to military families with school-aged children. Whether you are scheduled to arrive at Beale, are currently assigned here, or are about to PCS to a new duty station, the SLO is available to assist you.

Individual consultation can help parents understand how multiple PCS moves and deployments impact children in military families. Information is available regarding nationwide kindergarten and 1st grade entrance age requirements, a checklist of the documents required to enroll all school-aged children into a school, as well as local school district rules and regulations, schedules, calendars, and lists of curriculum and activities offered.

The SLO consults with teachers, community leadership and school officials about education, life, special needs & related issues to increase awareness of the unique challenges military children and their families face.

The SLO tracks updates on current and pending legislative initiatives that benefit school aged military children, and also provides links to online educational resources.

EXCEPTIONAL FAMILY
MEMBER PROGRAM
The Exceptional Family Member Program is designed to provide support to military family members with special needs. EFMP services include a variety of personnel, medical, and family support functions. The community support function of EFMP is provided by the A&FRC. That includes, but is not limited to, on-and off-base information and referral, parent training, support groups, relocation assistance, financial management, and school information.

TRANSITION ASSISTANCE PROGRAM
The Transition Assistance Program offers a monthly 3-day employment workshop sponsored by the Department of Labor and weekly congressionally mandated pre-separation briefings for members separating or retiring from the military. Monthly job search classes are offered for both the civilian and federal job sectors.

The TAP program also maintains various employment resources in the Discovery Resource Center such as books, newsletters, and magazines about resumes, cover letters, interviewing, and identifying a career that is suited to your personality type. Also, current job postings and job fair listings from both the surrounding areas of Beale AFB and nationwide are also available for jobseekers.

CAREER FOCUS
The Career Focus program offers group classes and individual appointments to assist in the job search process in both civilian and federal employment. Family members can find a variety of tools at the A&FRC to assist with career planning and development, with topics including resume writing, interviewing, self-assessment, networking, and education. Our goal is to provide family members with the resources and tools that will increase their success for employment in today's labor market, with an emphasis on portable skills and careers.

VOLUNTEER PROGRAM
Volunteers can register at the A&FRC to find out about the volunteer opportunities at Beale and in the local community. Anyone can be a volunteer; Airmen, retirees, family members, students, or DoD civilians with access to Beale AFB. Base agency volunteers qualify for 10 hours of paid childcare per week while they volunteer. The volunteer program is a great way for spouses to expand their networks, develop skills, and gain work experience in this tough economy. For the Airmen, volunteering is good for morale and can help bolster an individual's EPR or OPR.

PERSONAL & WORK LIFE
The Personal and Work Life program encourages families to develop and enhance their communication skills. Programs and classes focus on wellness and planning for change, helping build interpersonal skills necessary for family and personal growth.

MILITARY & FAMILY LIFE
CONSULTANT (MFLC)
The Military & Family Life consultant (a licensed therapist) is available for counseling with marriage or relationship issues, parenting issues, deployment issues, and daily life issues. These sessions are anonymous and no records are kept. Their permanent office is located within the A&FRC; however you can contact the MFLC directly on their cell phone (530) 601-6915.

CASUALTY ASSISTANCE
REPRESENTATIVE
Located in the A&FRC, the CAR provides dignified, compassionate and humane notification to active duty family members and other designated persons as promptly as possible when a member is placed in casualty status. The CAR also provides assistance to retired personnel, including change of address to pay records, change in federal and state income tax withholding allowances, answering questions on the Survivor Benefit Plan, and assisting spouses with the processing of all claims associated with the death of a retired military member. The CAR helps process the following forms: Emergency Family Member Travel (EFMT), Service members' Group Life Insurance (SGLI), Survivor Benefit Plan (SBP), Traumatic Service members' Group Life Insurance (TSGLI), Family Service members' Group Life Insurance (FSGLI), and Virtual Record of Emergency Data (vRED). DISCOVERY RESOURCE CENTER
The Discovery Resource Center (part of the A&FRC) provides walk-in customer access to computer software programs and public internet (including Wi-Fi connection), free printing, and access to scanners and a fax machine. The DRC houses a substantial library with hard copy resources, CDs and DVDs for individually-paced and self-directed exploration on all readiness subjects such as job search, financial planning, family issues, relocation and much more.

KEY SPOUSE PROGRAM
The Key Spouse program at the A&FRC educates and supports Key Spouses, who are volunteers appointed by their respective squadrons as a stable point of contact for family members. The Key Spouse program enhances mission readiness, and ensures family members receive regular contact from their squadron along with information and referrals to squadron, base and community resources.

SPONSORSHIP PROGRAM
Your sponsor is a personal representative of your gaining squadron commander and your new unit. They are trained and assigned to assist you and your family in all aspects of your relocation to Beale AFB. He or she will be knowledgeable about Beale and the surrounding communities and available to aid you from the time you receive notification of assignment to Beale until you are settled in your home and work center.

Do not hesitate to ask questions of your sponsor, as they have the resources of the squadron and the relocation assistance professionals at the A&FRC to help them address your concerns and needs. Remember, Beale is a relatively isolated base and unless you are arriving by car, you should coordinate transportation arrangements from the airport to the installation with your sponsor. AMBULANCE SERVICES
Emergency service is available 24 hours a day to respond to on-base illnesses/injuries, and is accessed through the 911 telephone system. Ambulance services provides medical coverage to after-hour in-flight emergencies and to various base events (upon request). After hours urgent or emergency medical care is provided by local civilian health care facilities.

CLINIC APPOINTMENTS
The Central Appointments Office books appointments for the Family Practice, Pediatrics, Flight Medicine, Women's Health and Optometry clinics. Dialing (530) 634-2941 accesses them. The appointment telephone lines are available Monday through Friday, 7 a.m. to 4 p.m.

FAMILY PRACTICE CLINIC
Outpatient medical care is delivered using the team concept. The team provides medical and preventive care for TRICARE enrollees (active duty, retired, dependent family members). A team consists of a provider, a nurse, and a medical technician who are responsible for managing the medical needs of approximately 1,250 enrolled beneficiaries.

Other responsibilities include conducting nurse-managed clinics under the supervision of the Primary Care Manager (PCM), performing patient triage (e.g., determining the severity of an illness and need for an appointment to receive medical care versus self care), administering medications and managing medical records.

PEDIATRIC CLINIC
The Pediatric Clinic provides comprehensive services to family members from birth to 11 years of age. This clinic addresses a wide range of medical issues, from the simple (e.g., physicals) to the complex (e.g., Synagis for babies at high risk for developing complications from RSV bronchiolitis).

WOMEN'S HEALTH CLINIC
The Women's Health Clinic provides comprehensive, personal care for women in all stages of life. Women's Health Clinic staff perform referral and consultative services including gynecology care, pap smears and clinical breast examinations. Obstetrical care is referred to a local civilian healthcare network through the TRICARE office.

PHYSICAL THERAPY CLINIC
The Physical Therapy Clinic offers comprehensive treatment and consultative services to patients who have been referred by their PCM team. Services include evaluation and treatment options for various musculoskeletal injuries. It also provides patients with information about their condition, appropriate exercise and therapeutic stretches that optimize healing.

MENTAL HEALTH CLINIC
Services are available for active duty personnel only. Non-active duty beneficiaries can receive these services through the TRICARE network in the local community. Mental health offers evaluation and short term treatment of depression, anxiety, combat stress/post-traumatic stress disorder and individual and family counseling, psychological assessment, group therapy, stress and depression management, substance abuse evaluations and social work services.

ALCOHOL/DRUG ABUSE
PREVENTION AND TREATMENT
Services are available and offer evaluation for alcohol abuse/dependency as well as alcohol/ drug abuse prevention education. Active duty personnel requiring inpatient services may be referred to Travis Air Force Base or the TRICARE network.

Family Advocacy offers short-term marital therapy, family maltreatment prevention counseling, New Parent Support Program (for parents with children under 3) and a broad range of family related classes and educational presentations to active duty and their family members. Patients may self-refer by calling (530) 634-3240 or be referred by their PCM, commander or chaplain. Further information may be obtained by calling (530) 634-3420.

IMMUNIZATION CLINIC
The Immunization Clinic provides a full spectrum of vaccinations to Active Duty, retired military, and dependents. They follow Centers for Disease Control and Prevention (CDC) guidelines to ensure vaccines are administered safely and correctly. Immunization programs include maintenance immunizations (e.g., tetanus, TB skin testing), well-baby vaccines, as well as smallpox and anthrax vaccinations. Allergy testing and immunotherapy are available on site.

PHARMACY
The pharmacy fills more than 10,000 prescriptions per month or 500 per day with an average wait time of 15 minutes for prescriptions entered by 9th Medical Group providers. This can take slightly longer for multiple prescriptions or those from civilian providers. Prescription service is available for all eligible beneficiaries. Please ensure you check in at the turn-in window to activate the refill process. The pharmacy may dispense up to a 90-day supply of chronic medication with refills for one year if requested by your provider and allowed by law. The pharmacy is open Monday through Friday, 7:30 a.m. to 5 p.m. Prescriptions from civilian providers will be honored if the prescribed medication is on our formulary. The Beale Clinic Pharmacy stocks all items on the DoD Basic Core Formulary (BCF) as well as many other medications. The formulary can be reviewed online at: http://www.pharmacyonesource.com/fos/formulary/beale/beneficiary or ask for a copy at the turn-in window. Prescriptions are refilled by using the pharmacy's 24-hour telephone refill service at (530) 634-2859 or (877) 471 4768. Refill requests called in before noon will be ready the next duty day by noon; requests after noon will be ready in two duty days by noon. LABORATORY
The laboratory is accredited by the College of American Pathologists. Limited laboratory services offered encompass most routine procedures used to treat a normal healthy population and most acute care scenarios. Many specialized tests can be performed through military reference laboratories; this averages seven to 10 days for receipt of results unless the provider determines a more urgent response is needed.

TRICARE network laboratories are used when other limitations dictate (e.g., specimen viability, transportation time, etc.). Questions may be directed to the laboratory staff at (530) 634-4450.

DIAGNOSTIC IMAGING
Services include routine radiological examinations and mammograms. All reports are interpreted off-site by Travis Air Force Base radiologists through a teleradiology program. Routine radiological exams are performed Monday through Friday with mammograms performed on Tuesday and Thursday. Questions may be directed to the diagnostic imaging staff at (530) 634-4810.

TRICARE OPERATIONS AND
PATIENT ADMINISTRATION
The Patient Administration office can assist you with items such as travel arrangements for medical TDY appointments, obtaining copies of your medical records, in/out-processing information as well as an array of other administrative functions.

To contact the Patient Administration office, please call (530) 634-4001. The Referral Management Center provides patients with specialty-care referrals, consultations and advice concerning treatment options. Questions about health benefits should be directed to the TRICARE Service Center representatives.

Our Beneficiary Counseling and Assistance Coordinator can help you make informed decisions regarding your healthcare needs by calling (530) 634-4817. The Patient Administration staff is available Monday through Friday, 7:30 a.m. to 4 p.m.

FLIGHT MEDICINE CLINIC
The Flight Medicine team serves the needs of our active duty flight crew members, air traffic controllers, special operational duty personnel, special occupational exposure personnel, select support personnel and associated family members. In addition to treating patients and providing preventive and self-care guidance, Flight Medicine responsibilities include responding to in-flight emergencies, performing shop safety visits/inspections, functioning as medical experts/consultants to all aerospace medicine duties, staffing two year-round deployed locations, and augmenting Safety and Accident Investigation Boards as needed.

DENTAL CLINIC
The Dental Clinic provides general dentistry services with some limited surgical capabilities to include intravenous conscious sedation of patients. The dental staff provides general dental care for active duty members. Retirees and dependents are limited to space available care only. Please call the dental clinic directly at extension (530) 634-4781/4782. After normal working hours or during weekends, emergency dental treatment is available.

Active duty family members can obtain dental care through enrollment in the TRICARE dental plan administered by United Concordia; call (800) 866-8499 for more information. Retirees can obtain dental care through enrollment in the TRICARE Retiree Dental Program; call (888) 366-3260 for more information. Dental questions may be directed to the Dental appointment desk at (530) 634-4781.

OPTOMETRY CLINIC
The Optometry Clinic treats most acute eye problems, to include diseases, infections (e.g., "pink" eye) and minor eye injuries. The clinic supports the War Fighter/Aviator PRK Laser Eye Surgery and Aviator Soft Contact Lens Programs plus provides "frame of choice" spectacles, gasmask inserts and specialized high altitude inserts for active duty members. Retirees with a current prescription may order spectacles.

Eye examinations are available for active duty and TRICARE Prime enrollees. Others are treated on a space-available basis. Additional services include flight line driver's license color vision tests, flying class physical eye exams and DoD Medical Exam Review Board physical eye exams. Appointments are scheduled through the Central Appointment office at (530) 634-2941. Retirees and their family members not enrolled in TRICARE Prime are seen on a space-avail- able basis.

PUBLIC HEALTH
Our mission is to prevent disease for all of Team Beale (Active Duty, contractors, civilians, and TRICARE Beneficiaries) whether at Beale AFB, deployed, or in the local community. We have two Elements: Community Health and Force Health.

In Community Health we conduct vector surveillance to mitigate vector borne diseases and control pest nuisances. Our job includes inspection of all public & food facilities on Beale AFB. We also provide food handler training for any food establishment supervisors, design Prevention/Health Education plans for all communicable disease situations such as sexually transmitted infections, flu outbreaks and Tuberculosis. We approve all temporary food establishments such as bake sales, barbeques and other fundraisers involving food sold or given away on Beale.

In Force Health our primary focus is in Occupational Health, Preventive Health Assessments and Deployment Medical Clearances. Public Health is the starting point for all medical deployment clearances of every AD/Mission Essential Civilian/Contractor.

Additionally, we are the base OPR for review of all mobility/duty restriction profiles (AF Form 469) and Overseas PCS/Security clearances. Public Health provides travel medicine briefings for any AD/dependant traveling to high disease risk areas as well as Medical Intelligence briefs for each deployment location. HEALTH AND WELLNESS
The Health and Wellness Center helps active duty military, retirees and their family members make healthier lifestyle choices by providing a wide variety of classes, pamphlets and videos. Courses offered encourage healthier eating, making better food choices, weight management, exercise, stress management, smoking/tobacco cessation, improving your fitness level, diabetes education, weight training, lowering your blood pressure, cardiovascular disease and men's and women's health. The staff can be reached at (530) 634-3500. These services are available to all active duty, retirees, family members and qualified base civilian personnel. The HAWC is located at 6101 B St.

BIOENVIRONMENTAL ENGINEERING
Bioenvironmental Engineering (BE) manages Industrial Hygiene, Readiness and Environmental Programs to evaluate and recommend controls for industrial operations that may have potential noise, chemical, radiological and biological health hazards, as well as collect potable water, industrial waste and environmental samples for analysis and interpretation. Other important responsibilities include consulting on radiation safety, planning prevention of and responding to weapons of mass destruction events, as well as involvement with the Fetal Protection Program. The results are used for indications of adverse trends for pollution prevention and ensure compliance with state and federal standards. BE is also responsible for the Industrial Respiratory Protection Program, the Gas Mask Quantitative Fit Test Program and Heat Stress Program. Information about community or work environments can be obtained by calling (530) 634-2045 during normal duty hours. The Bio- environmental Engineering office is located at 6601 B St.

AIRMEN'S ATTIC
The Airmen's Attic is staffed by volunteers from the community and offers members ranks E-5 and below used, serviceable appliances, furniture, household items, children's clothing and more free of charge. Items are not available to dormitory residents. Donations are accepted by calling (530) 634-5640 or DSN 368-5640. The program is located at 2459 Robert Nicoletti Way behind the Health and Wellness Center.

CHAPEL ACTIVITIES
The Beale chapel staff welcomes you and your family and invites you to join a vibrant and exciting chapel program. Their mission is to meet the spiritual needs of base personnel and their family members. They accomplish this by providing opportunities for worship, religious education, small group activities, outreach programs and fellowship events, all aimed at individual and family spiritual growth. There are two chapels at Beale. The Valley Chapel is located at 6199 C St. The Foothills Chapel is in the family housing area at 15001 Camp Beale Highway. Both chapels can be reached by calling (530) 634-4701. For information on off-base churches or Jewish, Orthodox, Muslim and Buddhist services, please call the chapel office at (530) 634-4701.

When problems arise for individuals and/or families, the chapel staff stands ready to help. The chapel staff offers broad personal and family counseling services and referrals when needed. Couples who are planning to be married are asked to contact the chapel as soon as possible. Regular duty hours at the chapel are 7:30 a.m. to 4:30 p.m. For off-duty emergency chaplain services, call the command post at (530) 634-5700.

BASE EXCHANGE
The Base Exchange is located at 17600 25th St., Building 2434. This facility offers authorized patrons a full assortment of retail merchandise. For more information call (530) 634-0200.

FINANCE
The Financial Services Office, located at 17855 Warren Shingle Road, is a customer service organization providing prompt and accurate pay support to military members and civilian employees. Behind-the-scenes activities include payments to commercial vendors and other government agencies for supplies, equipment and services and maintenance of accounting systems that track annual expenditures in excess of $60 million. Customer assistance is available on a walk in basis Monday through Friday, 8:30 a.m. to 4:30 p.m. and the cashier's cage is open 9 a.m. to noon. In-processing is held Monday and Wednesday at 8:30 a.m. in the Military Personnel Flight conference room. In-processing is scheduled through your unit's Commander's Support Staff.

If you plan on taking Permissive TDY for house hunting or leave after arriving, please in-process with our office first to prevent over/under payment of Military Pay entitlements. Members are required to in-process with finance no later than 15 days from their date of arrival at Beale. Travel settlements are typically paid within five workdays. In addition, members seeking additional funds to set up households may draw advance pay up to 180 days after arrival with their commanders' approval. Also, all members can use the government travel card for all official travel related expenses. Be sure to visit your departing FSO within 30 days of your departure so they can brief you concerning your PCS pay and travel entitlements and prepare your records for departure. CREDIT UNION
Sierra Central Credit Union operates out of three primary locations in the local area: the base branch, located at 17602 25th St., near the base exchange and commissary; the Yuba City branch located at 820 Plaza Way and the Marysville branch at 422 4th St. The credit union has a total of 15 branches in northern California. Additionally, it operates other offices in Red Bluff, Redding, Chico, Citrus Heights and at the Sierra Army Depot in Herlong, Calif. The credit union provides a broad variety of savings and lending services. They also provide share draft (checking) and Visa card programs. SEXUAL ASSAULT PREVENTION AND RESPONSE
The Sexual Assault Prevention and Response Office (9RW/CVK) is co-located with the Airmen and Family Readiness Center in Bldg 2179. This office provides the 9th Reconnaissance Wing and tenant units with sexual assault prevention education, self defense courses, and post assault response coordination. BASE ADVISORY COUNCIL
Another line of communication between the base populace and the Beale agencies is the base advisory council. The council is a sounding board for senior commanders on the base. Each unit at Beale has a squadron council and nominates representatives to the base advisory council. The goal of the council is to identify opportunities to improving base quality of life and investigate problems among base people and to investigate problems and research solutions dealing with many aspects of life.

SUPPORT FLIGHT CUSTOMER SERVICE ELEMENT
The Administrative Support Flight customer service element, located at 17852 16th St., is the focal point for obtaining military, dependent and civilian employee identification cards and information on personnel programs; Decorations, SGLI, MILPDS, Citizenship, UPRGs and Right Start. The customer service element provides customers the assurance that someone is available, cares and has the expertise to help resolve personnel problems. We are open Monday, through Friday, 7 a.m. to 4 p.m.

9TH FORCE SUPPORT SQUADRON, MILITARY
PERSONNEL SECTION
The MPS, located at 17855 Warren Shingle Road, is the single focal point for obtaining information on personnel programs and policies. It serves as the initial point of contact between the customer and MPS. When a customer visits the MPS Customer Support Element, a personnel professional will provide customers the assurance that someone is available, cares and has the expertise to help resolve personnel issues. The Career Development Element will provide assistance with PCS assignments, reenlistments, extensions, special actions, and promotions. The Force Management Element will provide assistance and expertise with evaluations, UIFs, duty titles, and duty statuses. VETERINARY TREATMENT
FACILITY
The Beale Veterinary Clinic provides service to privately-owned pets whose owners are enrolled in the DEERS program. We vaccinate new pets and update annual shots on older pets. Appointments may be scheduled weekly for routine vaccinations, and sick calls may be made for things such as ear mites, ear infections, skin checks and other minor problems. All pets must be registered within five days of being brought on base. Owners should have an identification tag with name and address placed on the collar. Owners must maintain current rabies vaccination status of their dogs, cats and horses. Registration and immunizations are obtained at the base veterinary clinic located at 17751 23rd St., Building 25504. Hours of operation are Mondays, Tuesdays, Wednesdays and Fridays, 9 a.m. to 12:30 p.m. and 1:30 p.m. to 4 p.m. and closed on Thursdays. Call (530) 634-2104 for an appointment. People living off base should check county or city laws for pet requirements.
