Blanchfield Army Community Hospital
On the installation, Blanchfield Army Community Hospital occupies more
than 494,000 square feet within the hospital and an additional 274,000
square feet of medical facilities outside the main hospital. The medical
services employees a workforce of approximately 1,480 military and
civilian employees, BACH provides quality health care and outstanding
customer service to approximately 102,161 active duty
and retired Soldiers and their Family members.
The Patient and Family Medical Resource Center
If you have questions about Blanchfield Army Community
Hospital services, programs or want to access the Internet
for reliable health and wellness information or take a
Self Care/Over-the-Counter medication class online, the
Patient and Family Medical Resource Center is an excellent
venue to assist you. You may stop by the Patient and
Family Medical Resource Center inside the "C" building
of the hospital in room 2BL01, Monday through Friday,
8 a.m. to 3 p.m. or call (270) 798-8055.
Information Desks
Information desk personnel provide information
about clinics and services. They can also assist in contacting
patients during their hospital stay. The information
desks are located at the "A" building main entrance
and the "C" building clinic entrance. The "A" building
desk is available 24 hours a day at (270) 798-8388. The
"C" building is available Monday through Friday, 7 a.m. to
3:30 p.m. at (270) 798-8410.
Admissions and Pre-Admissions
The Admissions and Dispositions Office is open 24 hours a
day, seven days a week and is located on the 2nd floor next to the
Emergency Center. All patients being registered, admitted or preadmitted
must have a valid form of military identification.
New Arrivals
Primary Care appointments are made by calling the Call Center at
(270) 798-4677 or (931) 431-4677 or by calling toll-free, (866) 524-4677 if outside
the calling area. The Call Center is open Monday through Friday, 6 a.m. to 6:30 p.m.
Appointments also may be made online at
www.tricareonline.com. Please have the following information available when you call to make an
appointment: sponsor's social security number,
sponsor's unit and work telephone number, and
names and birth dates of family members. If you
are unable to keep your appointment, please call
the appointment line and cancel as soon as possible.
You may leave a message to cancel any
clinical appointment after Call Center hours by
calling the appointment line.
TRICARE
TRICARE is the Department of Defense's
managed health care program for military service
members and their families. To enroll in
TRICARE, visit the TRICARE Service Center
located in Building 2525, 22nd Street and Indiana
Avenue. The center is open Monday through
Friday, 8 a.m. to 5 p.m. Questions about benefits
can be answered by calling (877) TRICARE,
(877) 874-2273 or by visiting
www.tricare.osd.mil.
Blanchfield Army Community Hospital falls in
the North Region and Health Net Federal Services
Inc. is the regional contractor providing health
care services and network-provider support in the
TRICARE North region.
All eligible TRICARE beneficiaries are entitled
to use Blanchfield Army Community Hospital,
although appointment availability will
depend on TRICARE plan enrollment. Active
duty Soldiers are automatically enrolled in
TRICARE Prime upon completion of the enrollment
application. TRICARE Prime is also open
to active duty family members.
TRICARE Prime patients receive the
majority of their care at BACH.
Family Member
Primary Care Clinics
Active duty family member primary care
clinics are designated by color—white and blue.
Children from birth until the age of 18 may be
enrolled in the Young Eagle (pediatric) Clinic
or Blue or White Clinics. All Fort Campbell
Soldiers and Family members will be assigned to
a Primary Care Clinic based on the Active Duty
spouse's major unit of assignment. This Soldier
and Family Centered Care team model will
enhance the continuity of care, enhance wellness
and readiness by improving communications
between units, families and the healthcare

Gold Primary Care Clinic
The gold clinic provides primary care services
with a focus on internal medicine to beneficiaries
enrolled in TRICARE Prime. The clinic is
open Monday through Friday, 7:30 a.m. to 4:30 p.m.

Specialty Clinics
Blanchfield Army Community Hospital also
includes a variety of specialty clinics such as
allergy, audiology, dermatology, otorlaryngology
(ENT), electrocardiogram (EKG), EEG,
immunization, sleep medicine, post traumatic
stress, traumatic brain injury, nuclear medicine,
neurology, nutrition care, obstetrics/gynecology,
occupational and physical therapy, orthopedic
surgery, pathology, preventive medicine, podiatry,
radiology and urology. For more information,

PTSD and TBI Clinics
The Post Traumatic Stress Disorder service
is located off 22nd and Indiana in Building
2524 and phone number is (270) 956-0593.
The Traumatic Brain Injury service is located
off 23rd Street and Indiana in Building 2543
and the phone number is (270) 956-0592. The
PTSD/TBI Clinics providers see patients on a
referral basis. The program provides intensive
TBI therapy, and it is important for patients to
attend all appointments for the best results.
Soldier Health Services
LaPointe Health Clinic provides quality
healthcare to Soldiers assigned to 1st, 3rd and 4th
Brigade Combat Teams and their Family members.
The clinic is open Monday through Friday,
6:30 a.m. to 3:30 p.m. except for Federal Holidays
and is located at 5979 Desert Storm Ave. (next to
the Soldiers Chapel). Services include primary care
services, audiology, chiropractic, physical exams,
nutrition care, physical therapy, laboratory services,
pharmacy services, radiology and optometry.

Aviation Health Clinic
The Aviation Health Clinic provides primary
care for Soldiers in the 101st and 159th Combat
Aviation Brigade and their Family members.
The clinic is open Monday through Friday,
6:30 a.m. to 3:30 p.m. except for federal holidays
and is located at 7149 Black Sheep Run. For

Carentan Primary Care Clinic
The Carentan Primary Care Clinic provides
primary care for Soldiers in the 2nd Brigade
Combat Team and their Family members. Carentan
is located adjacent to LaPointe Health Clinic.
Soldier Readiness Processing
Soldier Readiness Processing occurs at the
Dryer Field House which is located at 2604
Screaming Eagle Blvd. Medical Readiness is
necessary for all Soldiers. Soldier Health Services
is responsible for managing Soldiers' medical
readiness. Units needing to schedule specific
times may do so by calling (270) 798-5696.
Pharmacy Services
BACH Pharmacy
Location: Blanchfield Army Community
Hospital, 1st floor of the "B" building.
Services: BACH new or renewed prescriptions
-no refills
Hours of operation: Monday, Tuesday,
Wednesday and Friday, 7:30 a.m. to 6 p.m.;
Thursday, 8:30 a.m. to 6 p.m.; and Saturday,
8 a.m. to 4 p.m.
Town Center Pharmacy
Location: Building 98, in the former Pxtra
mini mall near the Commissary.
Services: The Town Center Pharmacy fills
off-post prescriptions, called-in refills and new
prescriptions.
Hours of operation: Will be Monday
through Friday, 8 a.m. to 5 p.m. and Saturday,
9 a.m. to 1 p.m. (called-in refill pick-up only).

PLEASE NOTE: All refills must be called-in
via an automated system, (270) 798-DRUG
(798-3784). Refills can also be requested via the

Patients unable to use the automated refill call-in
service may request refills by mail. Patients must
designate a desired pick-up date on the mail-in
card. Refills will be ready for pick-up two
working days after being called-in and refills not
picked up after seven days will be returned to
stock. All called-in refills can be picked up at the
Town Center Pharmacy location only.
Patient Advocate
The patient advocate serves as the liaison
between patients and the hospital staff. Comment
cards are available for compliments, suggestions,
inquiries and concerns. The representative can
also be reached by calling (270) 798-8091/8718.
All comments are documented and routinely
reviewed by the hospital commander. Compliments
are appreciated and shared with the staff,
as are concerns. Patients are encouraged to
contact the clinic chief, the ward noncommissioned
officer in charge or the head nurse to
resolve disputes or concerns. However, if the
matter cannot be resolved, a patient representative
will be happy to assist you. We thank you for

Beneficiary Counseling and
Assistance Coordinator (BCAC)
Beneficiary Counseling and Assistance
Coordinators educate beneficiaries and provide
a thorough understanding of TRICARE programs.

Debt Collection
Assistance Officer (DCAO)
Debt Collection Assistance Officers provide
assistance with debt collection matters concerning
unpaid TRICARE claims. Please call BACH's

TRICARE programs or unpaid TRICARE claims.
Fisher House
The Fisher House provides comfort and
care to patients and families receiving medical
treatment at Blanchfield Army Community Hospital.
The 7,200 square foot facility accommodates
up to eight families in individual suites
with private baths. Families share a kitchen and
spacious dining and living areas. The family
room is complete with a library and toys for
children. Soldiers and families enjoy in-room
telephones, cable TV, VCRs and computers with
Internet access. A laundry room offers an added
convenience for families during their stay. The
Fisher House is within walking distance of the

Army Substance Abuse Program
(educational and training program)
The Army Substance Abuse Program offers
substance abuse prevention classes, urinalysis
testing, risk reduction consultation for active
duty Soldiers, retired military personnel and
their family members. The program is located at
2526 22nd St. and Indiana Avenue and is open
Monday through Friday, 7 a.m. to 4:30 p.m. For

Army Substance Abuse Program
(counseling and rehabilitation)
The Army Substance Abuse Program (ASAP)
for counseling and rehabilitation offers community
counseling for alcohol and drug problems to
Active Duty Soldiers. ASAP receives command
referrals, post referrals, medical referrals and self
referrals. ASAP is located in Building 2526 at
22nd and Indiana, (270) 412-6883.
Sexual Assault Crisis Counseling
Sexual Assault treatment is available through
Blanchfield Army Community Hospital 24 hours
a day, seven days a week. If you have questions
about an immediate need of sexual assault
medical care, please call the Emergency Center at
(270) 798-8500. Victim Advocates are available
24 hours, seven days a week at (270) 498-4319.

Dental Headquarters
and Dental Clinics
The TRICARE Dental Program is a voluntary
dental plan available to family members of
all active duty and active National Guard and
reserve personnel.
Enrollment is a minimum of 12 months and
can be completed by calling United Concordia
at (888) 622-2256 to request an enrollment
form. Forms are also available online at
www.ucci.com. The dental activity headquarters
